If we want more people to get help for perinatal depression, removing co-payments for therapy might help. Study by @stephanievhall.bsky.social & co found enrollees in employer-sponsored plans who had perinatal mood/anxiety disorders had more therapy visits if their out-of-pocket costs were lower.

McEvoy CT, et al. Extended Continuous Positive Airway Pressure in Preterm Infants Increases Lung Growth at 6 Months: A Randomized Controlled Trial. Am J Respir Crit Care Med. 2025;211(4):610-8. The trial enrolled 100 preterm infants (24 to 32 weeks GA) who had at least 24 hours of CPAP and were considered ready to have the CPAP stopped (5 cmH2O, 21% O2, without tachypnoea or retractions or frequent apnoea, and tolerating breaks from CPAP during care procedures).

Difference between observed and predicted perinatal outcomes among U.S. birthing people prior to and after the COVID-19 pandemic. The plot represents the difference between the observed and predicted values for each outcome at each time point (month of birth), derived from Bayesian structural time-series analysis. The shaded area represents the 95% CI. The vertical dotted line represents the last month of the pre-period (i.e. March 2020 for prenatal outcomes and January 2020 for postnatal outcomes). The model includes a seasonal component to account for monthly seasonality and covariates for the proportion of participants aged 35 or more at delivery, the proportion of participants with more than high school level education, the proportion of White participants, the proportion of Hispanic participants, the proportion of participants using Medicaid for prenatal care, the proportion of primiparous participants, and average body mass index.

Interventions that are particularly effective in this age range: ⭐early childhood care and education (ECCE) ⭐parenting interventions ⭐cash transfers 1 yr ECCE would cost < 0·15% of low- & middle-income countries' current GDP. Cost of inaction is 8–19x the cost of investing in ECCE.
