I’m so happy that the @ash.hematology.org guidelines are getting a lot of attention but all these influencers who don’t actually understand iron deficiency posting about it is making me CRAZY.

THANK you my ferritin was in the toilet a few years ago - I think it was 14 when I realized I needed to test for it. never showed up in my hemoglobin numbers, but iron deficiency made me feel like I was dying very very slowly I fixed it with a hysterectomy tl;dr low ferritin w/o anemia is awful

On a personal note, my lowest ferritin was 9 (after a few, in retrospect, ill-advised RBC donations) and has only made it up to 26 after several months of PO supplementation. Very relieved insurance can’t give me/my PCP pushback on infusions with these new guidelines. AFAB folks take note!!

Landmark paper, with new iron deficiency diagnosis guidelines. Studies found that 27% of women have iron deficiency. Associated conditions (celiac, inflammatory conditions, kidney disease, heart failure) may be investigated and caught much sooner as a result. #anemia @shematologist.medsky.social

I hate that we've memory-holed 2020. My spring 2020 experience was 1) seeing ambulances pulling out dying seniors from a downtown Palo Alto center 2) the GOP's claim of "brutal lockdown" was remote school for only spring/fall 2020, with masked return in Jan-and that was far stricter than most places

A physician I know would spend their entire day in 2020 tending to people dying from COVID, then go home and have their parents tell them to their face that it was a hoax. When the doctor would try to share what they were seeing, they were dismissed and told they're just confused.

Back in 2020, as a pediatric hematologist married to a physician, we signed up to volunteer in a field hospital for patients with COVID. We wrote our wills and worried about getting our kids sick. I posted about terrifying outcomes in kids and received death threats. Fauci is a goddamn hero.

Dear insurance companies, If you don’t cover super important medications, the patient more times than not will end up in the ER and then the hospital and I’m pretty sure those stays cost a lot more than the medication you wouldn’t pay for. Sincerely, Your biggest hater

Wolfers: For nine out of the last 10 presidencies, if you’d said, “How much should we blame the president for what’s going on with the economy right now?” I would have said that the president is just one factor among many. That’s what I would have said nine out of 10 times. This is the 10th time.

PSA: Summer is a tough time for the blood supply as regular donors are traveling and schools (where drives occur) are out. At the same time, accidents requiring massive transfusion are more common. Only 3% of the population donates yet any of us could be in need someday. Donate if you can!

PSA: Many patients are waiting long times to see physicians, especially specialists. If you no show your appointment or cancel at the last minute, you are part of the problem. Things come up but it is oh so helpful for the whole system if you can cancel ahead of time or at the very least call.

Every week I see people who’ve had multiple visits, innumerable tests, failed medication trials & no one checked a ferritin. They’re always deficient & drastically better once deficit corrected. Iron deficiency screening in people who menstruate would be cost saving. And the right thing to do.

Here’s the truth about Medicare-for-all: You will pay less overall for better coverage. You won’t have to deal with a middleman company with a financial incentivize to deny as much care as possible. Your doctor won’t have to fight with that company to get you the care you need. It’s better.
