#medsky #episky 🧪 #pedsky #idsky Here's why HHS's demand for all new vaccines to be approved by placebo-controlled trials is actually profoundly unethical and jeopardizes all vaccines requiring rapid updates to cover pathogens that evolve quickly open.substack.com/pub/deplatfo...
HHS and FDA Demand Unethical Vaccine Studies
RFK Jr's. HHS is attempting to demand placebo-controlled trials in cases where it's not ethically appropriate.

The Washington Post recently broke the story that HHS, under the leadership of Secretary Robert F. Kennedy Jr., will be demanding that all new vaccines complete placebo-controlled trials before licensure. HHS’s comments therein note that Kennedy is not anti-vaccine, but rather “pro-safety”- a bald-faced lie that insults everyone’s intelligence. Similarly, FDA Commissioner Makary seems to be pushing for Novavax to conduct a randomized efficacy trial of their COVID-19 vaccine under the claims that it is a novel product because it has changed the strain of the spike protein it uses (it is not a novel product under existing regulatory framework and simply by common sense). I understand that headlines on this question have sparked confusion (e.g., “RFK Jr. will order placebo testing for new vaccines, alarming health experts”) and it may not be immediately clear why the health experts are alarmed. Here’s my best attempt to explain:

New viewpoint piece in JAMA concerning the early administration of MMR vaccine to children aged at least 6 months if they are at the site of a measles outbreak or traveling to one: jamanetwork.com/journals/jam... IMO:
Revising US MMR Vaccine Recommendations
Amid Changing Domestic Risks
Benjamin Rader, PhD, MPH; Rochelle P. Walensky, MD, MPH; Wesley S. Rogers, MD; John S. Brownstein, PhDRecent measles outbreaks within the US and worldwide necessitate a thoughtful examination of the US measles-mumps-rubella
(MMR) vaccination recommendations. The Advisory Committee on
Immunization Practices currently recommends 2 lifetime doses of
the MMR vaccine for most individuals, the first dose at age 12 to 15
months and the second between age 4 and 6 years.1
The committee outlines a few special cases for the administration of a third lifetime dose, including during an ongoing measles or mumps outbreak and among high-risk groups (eg, health care workers and
immunocompromised individuals).1
Additionally, for children aged
6 to 11 months scheduled to travel internationally, the committee
recommends an additional early dose (ie, a third dose) of the MMR
vaccine before travel, administered before initiating their routine
2-dose series at age 12 months.1
Historically, disease importations
from unvaccinated children traveling internationally to endemic regions has been themajor source ofUSmeasles cases.2However,multiple recent US measles outbreaks, coupled with low vaccination
rates, signal a growing domestic hazard.3-5
In some US states currently experiencing an outbreak, local advisories recommend infants aged 6 to 11 months receive an additional MMR dose. However, currently there is no national recommendation in place or guidance for infants traveling to these domestic
locations.6 Pediatric immunization guidelines should be revised in
light of evolving epidemiologic trends. We therefore propose updating the existing recommendation for an additional early MMR
dose to infants aged 6 to 11 months traveling to any region with increased probability of measles exposure, whether international
or domestic.
Current public health guidelines recommending an additional
MMR dose for infants traveling internationally are rooted in the understanding that measles exposure is a threat in some epidemic and
endemic settings worldwide. Unvaccinated travelers are at a higher
risk of be…among children younger than 5 years, with average coverage in the
South dropping below 70%.5However, these children have not yet
reached kindergarten, when immunization catch-ups typically occur to meet school requirements.
Reductions in US vaccination levels coincide with the recent increase in US measles outbreaks. In 2024, the US experienced 16
measles outbreaks totaling 285 cases and 114 hospitalizations. At
least 198 of these cases (69%) were related to a domestic outbreak.3
As ofMarch 7, 2025, there have already beenmore than 222measles
cases (at least 206 [93%] related to domestic outbreaks), 38 hospitalizations, and 2 reported deaths stemming from 3US outbreaks.3
Less than 5% of the 2024 and 2025 individuals with measles completed a 2-dose vaccination series.3
With some local immunization levels inadequate to avert
outbreaks3,4 and ongoing disease spread in various regions of the
country,3 a dichotomy between domestic and international travel
is not appropriate. For many travel itineraries, there may even be a
higher risk of measles exposure at the US point of departure than
at the international destinations. Additionally, much of the US remains highly vulnerable to measles outbreaks because of unvaccinated individuals who travel across state lines and introduce the virus domestically. This issue can be compounded by the social and
geographic clustering of vaccine hesitancy, which increases the likelihood that an unvaccinated traveler will return to a region with vaccination levels too low to contain onward transmission.2,5 Recognizing this potential risk, states such as New York already advise an
additional MMR dose for infants aged 6 to 11 months traveling internationally or to any domestic location experiencing a measles
outbreak.8 We endorse this policy and propose a national extension of these recommendations.
Many parents voice concerns regarding the cumulative number ofvaccinations administered during childhood, and anearly third
dose would be in addi…vaccination is performed before travel to locations with extremely
high measles exposure potential.2
Conclusions
The US should update its current MMR early third dose recommendation for infants aged 6 to 11 months and adopt a more flexible riskbased perspective. Current MMR recommendations should account for the shifting epidemiologic landscape and for clusters that
may appear domestically. Other countries may similarly wish to recommend an early MMR shot for currently unvaccinated infants traveling to locations in theUSwithmeasles outbreaks.Modernizingvaccination recommendations to reflectevidence-based riskcalculations
will better protect at-risk communities and the most vulnerable
US population—infants—against measles.
