Very sad. So many good doctors driven to either leaving the NHS or taking their own lives.

It is vital to act, and I urge colleagues to do so. How can we stand by and laud this man while he undermines scientific integrity? asks academic and author Kit Yates

Multiply this story across almost every hospital and Emergency Department across the UK. We need a proper review and an honest conversation about every aspect of healthcare in this country. This cannot go on. BBC News - 'No dignity in this', say staff on stretched A&E www.bbc.com/news/article...

Fantastic longitudinal #qualitative study by @oohgpwales.bsky.social, @trishgreenhalgh.bsky.social et al, examines challenges to quality in hybrid #PrimaryCare. Highlights the trade-off between accessibility & continuity, can lead to dehumanising & fragmented care. bjgp.org/content/75/7...

So this is the new great idea🤔 I’m pretty sure if Sarah had seen a GP she wouldn’t have needed to have a CT scan, no need to speak to the ENT registrar & yes it could be managed in the community Why does NHS England always underestimate the skills of GPs? www.england.nhs.uk/long-read/re...

There’s strong global evidence that what really improves health & mortality is continuity of care with the same healthcare professional. Rather than faff around with rebrands & with making sure it’s easier to choose private providers on the NHS app, this 👇 is what Streeting should be focusing on.

Context is key to understanding the pressures in the NHS 111 have 69200 calls a day A&E 72000 attendances Ambulance incidents 25000 GP practices have 1.4million consultations & several million more calls GP practices deal with 90% of demand & 20% more than 2019 @rthonwesstreeting.bsky.social

A brilliant editorial. Antipsychotic prescribing ⬆️s with no little change in incident psychosis. If the person took immunosuppressantdrugs for an autoimmune condition they would have lifelong structured monitoring (DMARD) shared between specialty & #primarycare =s discriminatory care.

Over Christmas, I did some reflections on the last few years. One theme that came up repeatedly is the bastardisation of "be kind" by people who want to use it to silence dissent. It doesn't seem like much considering the chaos of the last few years, but it has had a significant effect. 1/3

We have made it clear that the NHS temporary escalation spaces (TES) guide is "normalisation of what is an unacceptable and dangerous situation". RCEM Position Statement: tinyurl.com/rcemTES RCEM News: tinyurl.com/rcemNews1612
Ian Higginson, RCEM's Vice President, posts: "Called out NHSE’s narrative that it’s possible to provide safe care in corridors. Used strong language putting our statement together, reflecting RCEM members’ and colleagues’ strength of feeling on this matter. Degree to which corridor care is normalised is dumbfounding." What do they expect? With the lack of the nursing bursary, dire working conditions, 100k vacancies, low pay & sanctions for overseas workers, it's absolutely no surprise we are where we are. Without govt taking care of its staff, there can be no robust NHS. www.independent.co.uk/news/uk/home...

🧵 Whatever your views on assisted dying, I always assumed we’d all agree that the topic is ethically fraught & complex. Not so Matthew Parris. For him, it’s simple. Earlier this year, he argued that old & frail people *should* be killed - to save society the cost of looking after them. 1/n

ARRS will not be cut with the current NHS England board in place They think it’s working despite the 31k staff adding just 40m appts for £1.4bn Compared to 27k GPs doing 160m appts Cost-benefit not being done at the highest levels & they actually don’t know how it works #GeneralPractice

As a GP, I initiate conversations about limited life expectancy when appropriate. They are less initiated by secondary care teams and patients sometimes disagree with me- ‘my consultant is trying to build me up’. An important topic for discussion particularly for secondary/primary care interface.

If we are to rescue the #NHS , we need to start with rescuing #GeneralPractice first. Vast majority of patients contacts and #patientcare starts and ends in #TeamGP . No solution to the troubles of the NHS can be found without supporting it.

I’m not going to leave X just yet - I hate that sense of being driven away from a place I used to love. But after one day on here, I already know how much prefer it. So if you see this and would be kind enough to share it to let people find me, that would be hugely appreciated. Thank you.

Great piece in The Doctor @bmj.com on research by @trishgreenhalgh.bsky.social et al about whether quality suffers when GP working is ever more remote and fragmented. Traditional values still alive and well in primary care, but harder to realise them in the work.
