The US has at least 44 states and territories with COVlD outbreaks. The CDC made large, upward retroactive corrections to prior estimates, and we are now in an apparent secondary peak. States with observed outbreaks include Alabama, Alaska, Arizona, California, Colorado, Connecticut, Delaware, Florida, Georgia, Guam, Hawai’i, Idaho, Illinois, Indiana, Iowa, Kansas, Kentucky, Maine, Maryland, Massachusetts, Michigan, Minnesota, Mississippi, Missouri, Montana, New Hampshire, New Jersey, New Mexico, New York, North Carolina, Oregon, Pennsylvania, Puerto Rico, Rhode Island, South Carolina, Tennessee, Texas, U.S. Virgin Islands, Utah, Vermont, Virginia, Washington, West Virginia, and Wyoming. Additional states with poor monitoring likely have outbreaks based on high levels in bordering counties in adjacent states.

66 airport SARS-CoV-2 sequences from incoming travelers were uploaded today, collected Sept 21-30, and 26 (39.4%) were PJ.2.1. 20/30 (66.7%) sequences from Latin American travelers were PJ.2.1. Seems likely that PJ.2.1 is rapidly sweeping across the entire Western hemisphere.

A new variant has emerged that can bind our cells and infect us really well. So well that it means it can have further changes without losing its ability to infect and spread well. So… more waves, severity to be determined… exact interesting quirks to be discovered…

PMC COVlD Update, Sep 28, 2026 (US): 🔸We're in a 13th COVlD wave. 🔸40 states & territories continue to have COVlD outbreaks, stable from last week. 🔸Levels likely peaked between Sep 7-13 at a PMC-estimated 580,000 new daily infections. pmc19.com/map
The US has at least 40 states and territories with COVlD outbreaks, the same number reported last week. These include Alabama, Alaska, Arizona, Arkansas, California, Colorado, Connecticut, Florida, Georgia, Guam, Hawaii, Indiana, Iowa, Kansas, Kentucky, Massachusetts, Minnesota, Mississippi, Missouri, Montana, Nebraska, Nevada, New Hampshire, New Jersey, New Mexico, New York, North Carolina, Ohio, Oregon, Pennsylvania, Puerto Rico, Rhode Island, South Carolina, Texas, the U.S. Virgin Islands, Utah, Virginia, Washington, West Virginia, and Wyoming.
There are likely outbreaks in additional states. The summary statistic of 40 only includes counties with observed data, rather than estimated data. For example, Idaho, Louisiana, Maryland, and Tennessee likely have outbreaks, but monitoring is poor.

Nuance:
Note that while PMC uses forecasting to estimate national statistics through Sep 28 and beyond, the state-level data do not, meaning they are now 9 days old. If the national forecast is correct, levels peaked between Sept 7-13, closely aligned with our prior forecasts. This would mean that levels are declining more so than depicted in the map. Levels often decline slowly, suggesting the need for ongoing precautions. Additionally, Biobot and 200 CDC sites are offline this week, and both have experienced major retroactive corrections of late, so the national forecast has more uncertainty than usual. For example, we could see the peak adjusted upward or a double peak. It is unclear how quickly reporting will resume at the offline sites.

Note that while PMC national statistical estimates include forecasting, the state-level data do not, meaning they are now 9 days old. If the forecast is correct, levels peaked between Sept 7-13. This would mean that levels are declining more so than depicted in the map. However, Biobot and 200 CDC sites are offline this week and both have experienced major retroactive corrections of late, so high uncertainty.

I'll take this as a good-faith question and try to show which parts are fairly solid science, and where things have gone completely off the rails.

"So what changed?"- something did. We're in a kind of "my favorite planet is Pluto" kind of situation, but this time we can't just change what words mean in order to appease people who want what they learned in grade school to stay "true".

A long-held model of how most respiratory pathogens were transmitted was upended. For over 60 years, the theory was that exhaled, coughed, and sneezed droplets took a ballistic path- like a cannonball. They arced, fell, were picked up by hands, and then faces were touched, and infection resulted. If you had a little distance and washed your hands, it was felt the risk was minimal.

When a saltation variant occurs with COVID-19 it’s reported by variant hunters and we immediately reassure the public that it’s not going to be another pandemic even if it does go around the whole planet… just because it’s not as bad as when we first foolishly decided to let this thing spread.

Usually, when a disease is “endemic” countries have long standing plans to battle them. Targets, including zero targets. Now, we use the word to pretend we can ignore something. Yet another way COVID-19 is an exceptional disease.
