Poster title: optimising adherence to inpatient rehabilitation trial protocols: a mixed message systematic review

Background: millions of pounds are spent on research evaluating rehabilitation interventions for people after stroke but results can be negatively affected by endurance to the trial protocols.

Adherence is a key variable influencing the outcome of clinical trials.
Researchers need to identify ways to optimise adherence in the planning, design, and delivery of/rehab trials to ensure the funds are well used and that trials accurately determine treatment effects and patient safety

Methods: 
a mixed methods systematic review data was synthesised and integrated using the consolidated framework for implementation research 2.0 

summary of main findings: 27 papers included 17 of those in stroke, other conditions included neurological, surgical, intensive care, osteoarthritis, cardio respiratory, and acute medical elderly. 

The variability of adherence measures makes comparison between studies difficult, 63% of studies reported the number of sessions.
22 studies reported the duration of sessions e.g. minutes
Only 26% of studies used pre-specified appearance measures.

Multiple barriers and facilitators were identified across the domains of capability, opportunity and motivation.

Key messages: 
adherence to inpatient rehab trial protocols is multidimensional and multifactorial, spanning the innovation, the inner setting, outer setting, individuals, and the implementation.

Understanding factors affecting a appearance and how these interact with one another is key in the design, implementation, and reporting or future impatient rehab trial protocols 

Lack of consensus on appearance measures and levels makes meaningful comparisons between trials difficult.

Appearance level traffic light system would enable triallist to consider the needs to implement changes during the trial to improve an appearance or stop the trial.A scoping review of stroke services within the Philippines.

Access to stroke care & rehab services & the subsequent impact on stroke outcomes in the Philippines, a lower and middle income country, is unclear. Understanding gaps in service delivery & underpinning research from acute to chronic stages stroke will allow future targeting of resources. 

Methods: 70 papers were included. Data was extracted using structure, process, outcome.

Results:
Structure: gaps such as low number of neurologist, CT scanners were identified. Other gaps such as lack of/protocols and pathways, lack of infrastructure for emergency medical services, low number of acute stroke ready hospitals, Stroke Unit and rehabilitation facilities.
Potential solutions such as non-neurologist being trained to help stroke survivors, and mobile stroke units.

Process: gaps such as delayed arrival to hospitals, lack of training among health workers, poor community/awareness, lack of rehab for inpatient, outpatient and community.
potential solutions are use of telemedicine to reach geographically isolated regions, increase awareness of stroke symptoms and seeking emergency help, increase knowledge and importance of stroke rehab, TeleRehab.

Outcomes: gap such as number of stroke survivors with disability not reported, low government insurance coverage with high out-of-pocket expenses, limited data on carer strain & no data on Cara burden.
Potential solutions are to upgrade PhilHealth to cover reperfusion therapies, intra operable/registry representing real situations across rural & urban (disability as well as demographics).

Key messages
High prevalence stroke risk factors, high incidence of stroke and younger adult adults, TeleRehab developed in response to the COVID-19 pandemic but no data on implementation and sustainability, like all established/pathways and national registry,Barangay health workers’ knowledge about a key stroke is fair-good but knowledge and skills about drug rehab unknown.

I’ve started doing some private physio, having previously worked #NHS and the main impact on my day to day so far is the wonderful admin support. Notes software glitchy- let them know and it gets fixed. Appt making & changing - sorted. Lone working support that isn’t also out and about.
