Patients in rheumatology are at unique risk for premature aging, yet frailty is uncommonly accounted for in research or clinical care. Editorial in AC&R defines frailty through a rheumatology lens, providing guidance for frailty as a vital sign in care of individuals living with rheumatic diseases
Contributors to frailty in people living with rheumatic diseases and its impact on health outcomes. 
Frailty is a multidimensional risk state associated with decreased physiologic reserve and increased vulnerability to adverse health outcomes. Risk factors for the development of frailty are shown above (Box A).People living with rheumatic diseases may experience accelerated aging and premature frailty due to the presence of heightened systemic inflammation (especially in the context of poorly controlled disease activity), increased risk of sarcopenia, immunosenescence, immunosuppressive therapies, and irreversible organ damage. Frailty impacts the ability of an individual to respond to a new health threat or physiologic stressor, such as a serious infection or major surgery (Box B). A robust or non-frail individual will have the intrinsic capacity for full and rapid recovery from such an event whereas someone who is mildly frail may not fully recover in the face of a health stressor. Finally, someone who is more severely frail may not be able to withstand an additional health threat and may either die or lose functional independence as a result.

Today I learnt: The John Snow Pub on Broadwick Street in Soho, London was renamed as such in May 1955 to mark 100 years since the publication of Snow's masterpiece 'On the Communication of Cholera - Second Edition'. The name change and new sign was unveiled by a certain Sir Austin Bradford Hill.

A team of volunteer archivists has recreated the Centers for Disease Control website, called RestoredCDC.org, as it appeared the day President Trump was inaugurated. "I felt that we could restore any data sets that people might not have access to anymore because these were publicly funded data."

What’s happening with the US government reminds me of this story (great book btw): Libertarians took over a town in NH in 2004 and gutted its services, cut taxes, etc. Less than 8 yrs later things had gotten so bad that black bears were breaking into homes and killing people for the first time ever.
