Abstract

Background: The PACE trial was a well-powered randomised trial designed to examine the efficacy of graded exercise therapy (GET) and cognitive behavioural therapy (CBT) for chronic fatigue syndrome. Reports concluded that both treatments were moderately effective, each leading to recovery in over a fifth of patients. However, the reported analyses did not consistently follow the procedures set out in the published protocol, and it is unclear whether the conclusions are fully justified by the evidence.

Methods: Here, we present results based on the original protocol-specified procedures. Data from a recent Freedom of Information request enabled us to closely approximate these procedures. We also evaluate the conclusions from the trial as a whole.

ME/CFS Symptom Prevalence and Severity (These prevalence and severity figures are from A definition-based analysis of symptoms in a large cohort of patients with chronic fatigue syndrome, P. De Becker, N. McGregor, and K. De Meirleir. Journal of Internal Medicine 2001;250:234-240.) A total of 2,073 consecutive patients with major complaints of prolonged fatigue were assessed. Among them 1,578 met the Fukuda criteria and of those, 951 met the Holmes criteria. The figures indicate the differences in prevalence and severity of symptoms between these patient groups.

The HPA axis has been found to be dysregulated in people with ME/CFS 
Ongoing stress known as 'chronic stress' may lead to HPA axis dysfunction. 

HPA axis dysregulation may contribute to cognitive dysfunction through:

The idea that LLMs 'hallucinate' is itself a clever invention of the tech industry to disguise actual failures of their systems. Rebranding these serious algorithmic glitches as 'hallucinations' was designed to make the public accept this bullshit by linking it to the vagaries of human psychology.

Last-minute policy shifts by the Trump Administration propose to make the definition of medical frailty more restrictive, and make it more difficult for people to get exemptions from Medicaid work requirements. 

#MEAction is responding by pursuing parallel advocacy actions at both the state and the federal level. We will urge states to take all available steps to protect our community’s access to care under the new federal guidelines, while also pushing back that these federal guidelines need to be revised.

1) There’s an interesting lead in the ME/CFS genetic data: the eccentric medium spiny neuron (eMSN), a cell type in the brain discovered only a couple of years ago. All based on preliminary findings, but the data looks rather interesting.

I hate talking about the ballroom bc I don't give a fuck about the White House and nothing there is sacred to me, but it feels obvious to me that allowing an authoritarian, who attempted a coup the last time he lost an election, to build a "top secret" military complex under the WH is a bad idea.
