It's constantly changing, even the same patient with the same plan. I'll remember there was drama, go back and reread the notes, prescribe what they required last time, and they'll have changed their mind and want something different. I'm sure it's some PBM grift.

PA denied, try Dulera ("step therapy required"). Dulera is the same class of medication, not step therapy. And when I prescribe it, price estimate says $63. PA denial also notes the patient can use an Array Rx discount card instead of their insurance benefits.

I genuinely don't understand how the fuck we're supposed to work with insurance companies. I go to prescribe Symbicort. Price estimate says $50, PA required. I try Breyna, it says $16, no PA required. I prescribe Breyna. PA required.

aren't you just transmuting guilt from theoretically causing the rash to guilt for introducing multiple foods simultaneously? the multi-food exposure didn't cause the allergy either, and most babies won't have allergies to those foods. if they do have allergies, we can figure out which food it is.

Really interesting about lower medication adherence towards the end of a SNAP benefit cycle -- even though they already had the medication, worries about running out of food seems to interfere with the automaticity of daily meds.

I watched a child die of tetanus on one of the more remote islands in Vanuatu. They have a vaccine program but the village's vaccine fridge was broken. It was awful, anguishing, and absolutely radicalized me.

Splitting three vaccines into one was a key part of Wakefield's grift back in the day, and wasn't motivated by science or health or caring about children, but by the fact that he had a financial interest in the single vaccines and wanted to sell them. That's the basis. Not health. Money.
