A County-Level Collaboration to Reduce Infant Mortality: Cradle Cincinnati
10.1542/6401842270112Video AbstractPEDS-VA_2026-0771166401842270112OBJECTIVE. We sought to implement and determine the efficacy of Cradle Cincinnati—a county-level collective impact collaborative established to reduce the infant mortality rate (IMR) of Hamilton County, Ohio. We hypothesized that the reduction in the IMR of Hamilton County was attributable to the work of Cradle Cincinnati.METHODS. Cradle Cincinnati, conceptualized in the early 2000s and iteratively implemented in the ensuing decades, emphasizes stakeholder engagement, data-driven learning, and community-led strategies to address infant mortality. To determine efficacy, we employed augmented synthetic control methodology to create a synthetic Hamilton County unexposed to Cradle Cincinnati using county-level data from Ohio in the years 2007 to 2023, with change in county-level IMR in the real Hamilton County compared to a synthetic unexposed Hamilton County as the outcome of interest.RESULTS. Hamilton County IMR decreased from 11.1 (per 1000 livebirths) in 2008 to 5.5 in 2023. Real Hamilton County IMR progressively decreased over time compared to that of a synthetic Hamilton County unexposed to Cradle Cincinnati, reaching statistical significance in 2021 and 2023 (change in deaths per 1000 live births: −2.5 [95% CI: −5.1 to −0.3] and −4.4 [95% CI: −6.6 to −2.0], respectively).CONCLUSION. Cradle Cincinnati significantly reduced infant mortality in a large urban Ohio county, with the greatest improvements emerging nearly a decade after implementation. A county-level focus offers potential to reduce the disproportionately high and racially disparate IMR in the United States.

Nature Reviews Immunology, Published online: 18 September 2026; doi:10.1038/s41577-026-01354-7Immune plasticity during the perinatal period enables mammalian offspring to successfully navigate the transition from prenatal, intrauterine life to postnatal, extrauterine life. This Review highlights recent advances in our understanding of perinatal immune plasticity and the factors that modulate it, with implications for neonatal health and disease.

Routine vitamin K prophylaxis. Why are there so many refusals?
This is still a fairly rare phenomenon in my hospital, but it does seem to increasing slightly here, as it is just about everywhere else. The science is settled, transplacental transfer of vitamin K is extremely limited, hepatic stores at birth are very low, and human milk contains extremely low concentrations of vitamin K. In later life, most of our vitamin K2 is produced by intestinal microbiota, including by our friends the Bifidobacteria, as well as Bacteroides and Prevotella, but we don't have many of those present at birth or over the first few days.

Pediatrician Dr. Mildred Stahlman revolutionized the care of premature infants (incl a miniature iron lung). She spent her career at #Vanderbilt U. Med Center & is credited w/ establishing the world's first modern Neonatal Intensive Care Unit (NICU). Born #OTD in 1922. #WomenInSTEM #MedSky (1/3)🧵
A black-and-white historical photograph of Dr. Mildred Thornton Stahlman (1922–2024), the visionary pediatrician who founded the world's first modern Neonatal Intensive Care Unit (NICU). Seen from the waist up, Dr. Stahlman stands in a hospital setting, cradling a newborn infant gently but securely in her hands. She wears a protective medical scrub gown over a collared shirt and tie, her hair neatly pulled back, revealing a focused, deeply compassionate expression as she looks down at the baby. The infant, wrapped in a small cloth, rests fragilely against her forearms. The background is a softly blurred hospital nursery or NICU, featuring medical equipment, charts on a bulletin board, and another figure indistinctly visible, emphasizing her daily clinical environment. The composition is intimate and tightly framed, casting a poignant, reverent mood that highlights her lifelong dedication to pioneering neonatal care and saving infant lives.

Table 4: Clinical outcomes of infants receiving PDA treatment (n=1097) vs. conservative management (n=259). PDA treatment associated with lower predischarge mortality (14.5% vs 15.4%, AOR 0.35) but higher moderate-severe BPD (55.2% vs 32.4%, AOR 2.11) and NEC ≥2 (11.6% vs 6.2%, AOR 2.05).
