Agreed. It's time to stop the protein-restriction nonsense in early CKD. First, do no harm! If a compelling, long-term RCT demonstrates a clear benefit of protein-restriction, we can talk. The burden of proof is on the intervention. The proof is missing, and has always been missing.

Agreed. For me the answer to your question is easy: opt for the conservative, preventative approach and advise limiting K consumption. The other option is like prescribing an antidote to rat poison rather than telling the patient to quit eating the rat poison.

Clean keto has actually a better K/Na ratio than the standard American diet. It’s because you eliminate ultra processed foods. You also eat plenty of nonstarchy veggies.

Here is a study comparing very low carb vs Dash diet. I think there are a few more similar ones. We see potent effects of VLC/keto diet on hypertension in ADPKD and there are lots of studies in T2D etc. doi.org/10.1370/afm.....

Hmmm, why not just skip the Dash diet? It doesn’t help with BP anyway. Very low carb is much more effective and can be done with low K for those with hyperK problems. Then you don’t need K binders either
