The next stages of the SPR must incorporate people in the last year of life and include the option to complete advance care planning within the delivery of the NHS App.

Good to hear @sharonbrennan.bsky.social @nationalvoices.bsky.social championing the crucial role that VCSE’s will play in realising the shift to community and brilliantly making the point that the 10YP care plan target must not result in tokenistic recording of patient wishes. #LabourConference2025

This is a great example of what we do at Compassion in Dying - find practical solutions to help people navigate decisions when they're not being listened to. We see lots of examples that show how policy is good but the gap between that and practice can be vast.

This is such an important point. We can't 'sort' dying just by investing in services (important though that is). We need to invest in community: its understanding, choices and agency.

We need to make sure what comes out of the 10 year plan for end of life care includes a new focus on listening to and foregrounding the wishes of people. 👀 Some thoughts from me 👇

New findings from @mariecurieuk.bsky.social @nuffieldtrust.bsky.social and Health Economics Unit make for an important read. Current health spending for people in their last year of life is disproportionately distributed towards hospital care rather than primary and community care services.
