Could a brief speech sample support primary progressive aphasia (PPA) diagnosis? Researchers at the UCSF Fein Memory and Aging Center found that automated analysis of a 1-2 minute picture description accurately distinguished PPA variants and aligned with known brain and pathology patterns.
Three side-by-side images of the left side of the brain show areas where speech patterns are associated with different forms of primary progressive aphasia (PPA). Colored regions (blue to red, with warmer colors indicating stronger associations) highlight different brain areas for each subtype. Nonfluent PPA is associated mainly with the left frontal regions involved in speech production. Logopenic PPA is associated with the left posterior temporal and parietal regions involved in language processing. Semantic PPA is associated with the anterior temporal lobes, especially on the left, which support word and concept knowledge. These images illustrate that each PPA subtype is linked to a distinct pattern of brain changes.

Our new paper in @jamaneurology.com shows that automated analysis of just 1–2 minutes of speech can accurately distinguish primary progressive aphasia variants and reflects their underlying anatomy & pathology, with potential as a practical, interpretable tool to aid diagnosis and disease monitoring

Can FTLD-TDP and LATE-NC be reliably distinguished? Researchers at UCSF Fein Memory and Aging Center analyzed one of the largest FTLD-TDP brain bank cohorts and found that, despite important clinical and pathological overlap, the evidence supports these as distinct disease entities.

The first four panels show tissue from the middle frontal gyrus stained with H&E and TDP-43, highlighting abnormal TDP-43 protein deposits inside nerve cells and their processes. The remaining panels show a FTLD-TDP type A case with a greater burden of TDP-43 pathology in other brain regions, including the subgenual cingulate, entorhinal cortex and superior temporal gyrus. The figure illustrates that examining only the middle frontal gyrus may not reliably distinguish FTLD-TDP type A from LATE-NC.

Tau PET scans help clinicians visualize Alzheimer’s disease biology in living patients. But sometimes scans “light up” for reasons other than tau tangles. The findings could help clinicians interpret scans more accurately and guide diagnosis, prognosis and Alzheimer’s drug trials. bit.ly/4ll0n6A
