To our knowledge, this is among the few examples where the evolutionary history of a pediatric cancer could be reconstructed from gametogenesis to tumorigenesis. Many thanks to all co-authors and collaborators for helping unravel this molecular puzzle. doi.org/10.1016/j.jh...
Overview of the study

Theo Hirsch @tz-hirsch.bsky.social · 06/07/2026
Trisomy 18 is a recognized risk factor for hepatoblastoma, but the responsible locus has remained unknown. Because only part of chromosome 18 was duplicated, we could substantially narrow the candidate region associated with HB predisposition.
Complex rearrangements of chromosome 18 resolved through WGS and specific FISH

Theo Hirsch @tz-hirsch.bsky.social · 06/07/2026
The rearrangement was present in only ~10% of normal liver cells and blood cells, yet in 100% of tumor cells. This allowed us to reconstruct how developmental mosaicism evolved into a clonal hepatoblastoma.
Phylogenetic evolution focused on copy-number alterations, from gametogenesis to tumorigenesis

Theo Hirsch @tz-hirsch.bsky.social · 06/07/2026
How do we know the rearrangement arose before fertilization? Haplotype analysis revealed three chromosome 18 haplotypes, supporting a pre-zygotic origin of the rearrangement. Mosaicism therefore resulted from embryonic rescue rather than from a post-zygotic event.

Theo Hirsch @tz-hirsch.bsky.social · 06/07/2026
Most chromosomal mosaicisms arise after fertilization. We report a child with hepatoblastoma carrying a chromosome 18 rearrangement that instead originated before fertilization. Multi-omics analyses revealed its evolution from gametogenesis to cancer. doi.org/10.1016/j.jh...
