I'm talking with a patient from the Otsuka/Transcend methylone phase-II trial. They said there it essentially included therapy. The drug was presented as something that would help them process emotions/memories, like MDMA does. They planned to address certain emotions/memories before each session.

Questioning the recovery of dissociated traumatic memories under psilocybin: comment on “Therapeutic emergence of dissociated traumatic memories during psilocybin treatment for anorexia nervosa” - Journal of Eating Disorders
In their recent case report article, Peck and colleagues suggested that two patients recovered dissociated traumatic memories during psilocybin treatment for anorexia nervosa. These case reports are of clinical and scientific interest and confirm that psychedelics may induce vivid memory-like experiences. However, the reports warrant scrutiny. Here, based on what is known about recovered memories and the effects of psychedelics, we argue that the authors may not have adequately considered alternative explanations. The cases do not necessarily demonstrate that psilocybin induces recovery of dissociated traumatic memories or could treat dissociative amnesia. We further caution against the authors’ suggestion of explicitly preparing patients for the emergence of forgotten material.

Classical randomized controlled trials (RCTs) can't capture the complexity of psychedelic therapy. In this new paper led by @sdmuthu.bsky.social and @tehseennoorani.bsky.social, we argue for pragmatic trials designs that accommodate this complexity and better align with real-world clinical practice.

Reframing Psychedelic Regulation: Tools, not Treatments In this new article, we propose to regulate psychedelics as psychotherapeutic treatment tools rather than as treatments in their own right. The use of anesthetics as treatment tools in surgery is a useful analogy.

Surprising and concerning that though the authors do mention the importance of "not guiding towards confirmation or denial", they have no problem declaring (including in the title of a scientific paper!) that for both these two patients it was in fact "emergence of dissociated traumatic memories".

An important empirical question is how belief in the "reality" of encountered entities relates to outcomes. My guess is that belief magnifies negative outcomes and is less clearly related to positive outcomes, but that needs to be tested. Results could inform best practices for prep & integration.
