Reassuring evidence: trauma-focused therapy for young people with PTSD appears very safe. There is no evidence that direct trauma processing causes harm. acamh.onlinelibrary.wiley.com/doi/10.1111/...

7/7 Our take-home message: Effective psychological treatment and treatment safety are not opposites. The available evidence is reassuring—but we need better and more consistent assessment & reporting of negative outcomes in future research. 📄 Read the open-access paper: doi.org/10.1111/jcpp...

6/7 So what does this mean for families and clinicians? Overall, the evidence suggests that psychological treatments for young people with PTSD—especially TF-CBT, for which we have the most evidence—are safe and acceptable for the vast majority of children and adolescents.
110
Thole Hoppen @tholehoppen.bsky.social · 10/08/2026
5/7 There is an important caveat, though. Some negative outcomes are assessed rather seldomly. Only 6 of the 72 trials reported deterioration of PTSD, and only 19 reported adverse events. That means the evidence on deterioration and adverse events is less certain than we would ideally like.
110
Thole Hoppen @tholehoppen.bsky.social · 10/08/2026
4/7 We also looked at adverse events—significant negative events occurring during the treatment phase. These were very uncommon: 🔹 0.29% during psychological treatment 🔹 1.14% during waiting-list control There was no evidence that psychological treatment increased the risk of adverse events.
110
Thole Hoppen @tholehoppen.bsky.social · 10/08/2026
3/7 What about the concern that trauma-focused therapy might make PTSD symptoms worse? The available evidence is reassuring. Among studies of TF-CBT, less than 0.01% of young people showed a worsening of PTSD after treatment completion. In waiting-list controls, 1.26% of patients got worse.
110
Thole Hoppen @tholehoppen.bsky.social · 10/08/2026
2/7 First, treatment dropout was relatively uncommon. About 8% of young people receiving psychological treatment dropped out before the end of treatment when compared with waiting-list controls. Importantly, dropout rates were not higher than in control groups (e.g., passive or active).

Thole Hoppen @tholehoppen.bsky.social · 10/08/2026
🧵 1/7 Are psychological treatments for children and adolescents with PTSD safe? We just published a new systematic review and meta-analysis looking specifically at this question. We brought together evidence from 72 randomized controlled trials involving 5,677 young people. Here’s what we found👇

These findings show that PTSD-specific psychotherapies—especially TF-CBT—don’t just ease PTSD, they also substantially reduce comorbid depression, which is ecouraging for clinical practice. • Rates of response/remission for TF-CBT were 3× higher than passive controls. • Other interventions showed similar effects relative to controls. Yet, data was scarce for other interventions, particularly mid- and long-term data. More research (incl. long-term assessments) is needed. 4/5

Our analysis of 136 RCTs (N=8,868) found that: • Most evidence accumulated for Trauma-focused CBT (TF-CBT) • TF-CBT led to large reductions in depression vs passive controls (g=0.97) and moderate vs active controls (g=0.50) • TF-CBT outperformed non-trauma-focused therapies in various analyses 3/5

However, more (rigorous) studies are needed to increase statistical power of future meta-analysis and certainty of evidence. Future studies need to investigate long-term behavioural impacts. For the time being, long-term data of behavioural change remains scarce in the field of SC-BCTs. 6/X
