Aaagh WTAF?!? 😱 I knew IPC's delusions of adequacy were bad, but...they hand-waved "evaporates to droplet nuclei that remain airborne indefinitely" into "evaporates, so that means it magically disappears and it can't be airborne" How is medical leadership so full of people who can't 🤬 THINK?!?
Studies by ventilation engineers
These studies fall into another category of research but it is always worthwhile to link expertise in different fields together. There is evidence that most of the droplets produced by a patient will evaporate totally. Xie et al. reviewed the literature and estimated that ∼1% of particles from the mouth were larger than 100 μm.20 They plotted an evaporation curve and showed that a particle of such a size would be fully evaporated in 1.5 s.21 Thus, unless the host was intimately close, cross-infections might not be that easy by air, suggesting again that airborne transmission may not be a frequent occurrence.

An absolutely damning report on the unnecessary COVID deaths of workers at the Cargill plant in High River, AB, highlighting public health and infection control's mismanagement of the crisis, and failure to engage OHS or follow the precautionary principle. albertapolitics.ca/wp-content/u...
As the evidence marshalled in this report shows, the massive
outbreak associated with Cargill was by no means inevitable. It
occurred because systems designed to protect worker health and
safety failed. And they failed for a number of interrelated reasons.
OHS and AHS Did Not Adopt the
Precautionary Principle
The virus SARS-CoV-2 is spread among humans via aerosol
transmission. The period during which someone with COVID-19
can transmit the virus varies depending on factors such as the
variant and severity of illness, but transmission can occur both
before and after symptom onset. Some infected individuals never
experience illness but still spread the virus to others.
At the beginning of the pandemic, Hinshaw and many other
public health leaders believed that COVID-19 only spread via
large droplets and that asymptomatic transmission was not
occurring. As a result, AHS EPH primarily focused on physical
barriers, physical distancing, hand-washing, and surgical masks as
preventative measures at Cargill.Cargill, AHS, and OHS did not consider the possible range of
aerosol transmission of the virus in a large crowded industrial
work setting and, therefore, did not prioritize — let alone consider
— higher-level control measures such as improved ventilation, air
filtration, and N95 (or equivalent) respirators.
Surgical masks, which were made mandatory at Cargill after April
16, provided inferior protection to workers working eight hours a
day, especially for those working in close quarters with inadequate
ventilation. Sometime between the closure of the facility (April
22) and its reopening (May 4), Alberta OHS came to believe that
tight-fitting, higher-filtration respirators (N95 or equivalent
masks) were required, as demonstrated by the ministerial order
approving KN95 masks (May 3) and OHS officers’ decision to wear
N95 masks during their May 4 inspection. However, the evidence
suggests that this view was not shared by Hinshaw.
Ventilation was not identified as a potential factor in the spread
at Cargill until early May, and even then, there is no evidence that
this pathway was further investigated by AHS or OHS.
Had OHS, AHS, and Cargill adopted the precautionary principle,
which requires taking proactive measures to maximize human
safety before the science about the transmission of a new virus
is known, more aggressive control measures would have been
adopted early on. If improved ventilation, provision of N95 masks
(or cartridge respirators), and other actions had been taken earlier,
hundreds of workers may have been spared from illness.None of the evidence we reviewed showed any communication
or coordination between AHS EPH officials and Alberta OHS at
this time. In response to our freedom of information requests for
email correspondence between the manager for OHS in Calgary
and several AHS officials who were directly involved in the Cargill
response, we were informed by the Government of Alberta’s
access to information that “a thorough search […] did not yield any
records responsive to your request.”195
By April 16, the total number of known confirmed positive cases
had reached approximately 550. At this point, there had only
been one in-person inspection of the plant, that being AHS’ April
7 visit, which resulted in non-binding recommendations. And, as
mentioned, there was no epidemiological analysis of the outbreak
showing the temporal pattern of the spread within the plant and
in the community.
An internal AHS MOH update on April 17 noted that “84% of
new COVID-19 cases [in the province] are in Calgary Zone, ‘most
related to Cargill.”196 By this time, a Cargill worker and union shop
steward had been admitted to an ICU. On April 18, another worker
would need life support care in the ICU. Cargill, AHS, and OHS had
lost control of the outbreak.Echoing Cargill’s response to the union’s April 12 letter, Dreeshen
cautioned workers not to listen to “inflammatory rhetoric” and
appealed to them to instead focus on information from public
health experts:
“…On this call are Alberta’s best medical minds and
they provide Alberta’s government with the best up
to date information on a daily basis and I would just
like to encourage everyone to not get distracted by
misleading noise or inflammatory rhetoric because
any government's primary goal — our core goal — is
to keep our people safe…”
Immediately after the town hall, Dreeshen posted on his social
media: “Directly communicated with [Cargill] workers to reassure
them that their worksite is safe.”217 Internal email messages
among senior officials with the Ministry of Agriculture and
Forestry suggest there was also “a request” to share some “key
messages” about the safety of Cargill’s plant through Hinshaw’s
Twitter account.218
Hours after the town hall, Dr. Nick Etches, acting lead medical
officer of health for the Calgary Zone, wrote to colleagues
requesting assistance with an epidemiological analysis of
the outbreak:
“[Hu] just spoke with [Hinshaw] and she wants us
to answer some questions that will likely require
additional epi support. Essentially we want to
determine the proportion of the Cargill-associated
cases that are due to:
1) Workplace transmission

Why is this research being done?

The International Agency for Research on Cancers, part of the World Health Organization, has found that working as a firefighter can increase the risk of cancer.

In response, the Government of Canada passed Bill C-224. This bill created a national plan to raise awareness about cancers linked to firefighting and to improve access to prevention, screening, and treatment.

As part of this work, Health Canada selected McMaster University’s Cochrane Canada and GRADE Centre to develop guidelines for cancer detection in firefighters. We will develop about 20 recommendations, …

I hand out respirator/breatheteq masks to my patients to help them reduce their risk of getting COVID and Long COVID. Here's the letter I've put together about this for parents about children masking. @longcovidresource.bsky.social @longcovidadvoc.com @longcovidkids.bsky.social #longcovid
Letter with different typeface colours entitled, "Preventing Long COVID in Children". Text as follows:

As described by the Public Health Agency of Canada1,
Long COVID (also known as post-COVID-19 condition) affects children as well as adults.
- It’s long-lasting symptoms after a known or unknown COVID infection.
- The most common Long COVID symptoms in children are:
Fatigue
Headache
Abdominal pain
Sleep problems
Shortness of breath
Cognitive problems (difficulty thinking or concentrating)
Muscle aches and joint pains

I care for children with Long COVID. It can interfere with their social, physical and educational development. You do not want your child to develop Long COVID.

While there are some promising therapies, there is no clear cure for Long COVID.

The best way to prevent Long COVID is to prevent COVID infections.
Since COVID is airborne, the best way to prevent COVID infections is to wear respirator-type masks (like N95s) all the time indoors. This is why I’m giving you this mask for your child to wear. I personally purchased these to hand out to my patients. This is how important it is to me, as your child’s doctor, for your child to mask: to protect their health.

I know there will be times at school and other places where your child will want to remove their mask. Wearing a mask as much as possible in public indoor spaces will greatly reduce your child’s risk of getting COVID, which will reduce their risk of developing Long COVID. If they wear it 90% of the time, that’s still a huge win and way better than wearing it only 20% of the time, or not at all.

> Key tip: shape the curve of the nose piece first, then put the mask on.

So for anyone in Ontario wondering why infection control practice is so backwards here even away from the usual suspects, it turns out there's misinformation embedded directly in the CPSO infection control guidance, which incorrectly directs physicians to wear a medical mask for airborne pathogens.🙄
Physicians must undertake Routine Practices and Additional Precautions4, including:
hand hygiene for patient and physician safety, in accordance with Appendix B: Ontario’s Just Clean Your Hands Program: Your 4 Moments for Hand Hygiene and Appendix C: Hand Hygiene Methods;
wearing gloves if there is expected exposure to blood, body fluids, secretions, skin rash or non-intact skin;
wearing a medical mask when required to prevent transmission of infectious agents in droplets and airborne particles, and during aseptic and invasive procedures; and
wearing eye protection and a gown when required to protect eyes, skin, and clothing from blood, body fluids, secretions, or excretions.

While the majority of health care providers advocated for mask use, a minority of health care providers offered contradictory guidance. One physician stated:

I’m a physician—I believe in statistics—I follow the science. There is no evidence that masks decrease the spread of COVID-19. Masks cause issues for children ...

reasonable evidence of an impending threat to public health, it is inappropriate to require proof of causation beyond a reasonable doubt before taking
steps to avert the threat. As an editorial in the American Journal of Public
Health in May 1984 put it:
The incomplete state of our knowledge must not serve as an excuse for
failure to take prudent action. Public health has never clung to the principle that complete knowledge about a potential health hazard is a prerequisite for action. Quite the contrary, the historical record shows that
public health’s finest hours often occurred when vigorous preventive
action preceded the crossing of every scientific “t” and the dotting of
every epidemiological “i”.
Perhaps the best indication of the Red Cross’s lack of appreciation of the
risk of transmitting AIDS through blood was expressed in July 1985, when
Dr Davey said that if HIV-antibody testing had been carried out for the previous five years, only three cases of transfusion-associated AIDS would have
been prevented. He was wrong

The Commission therefore recommends: • That the precautionary principle, which states that action to reduce risk need not await scientific certainty, be expressly adopted as a guiding principle throughout Ontario’s health, public health and worker safety systems by way of policy statement, by explicit reference in all relevant operational standards and directions, and by way of inclusion, through preamble, statement of principle, or otherwise, in the Occupational Health and Safety Act, the Health Protection and Promotion Act, and all relevant health statutes and regulations. • That in any future infectious disease crisis, the precautionary principle guide the development, implementation and monitoring of procedures, guidelines, processes and systems for the early detection and treatment of possible cases. • That in any future infectious disease crisis, the precautionary principle guide the development, implementation and monitoring of worker safety procedures, guidelines, processes and systems.

Fundamental flaws in the UK’s approach
1.44	Despite knowledge accumulating of how Covid-19 may have been transmitted as the pandemic progressed (as set out above), insufficient steps were taken to adapt the infection prevention and control guidance for healthcare settings across the UK. This placed healthcare workers and patients at greater risk.
1.45	The UK’s approach to infection prevention and control in healthcare settings during the Covid-19 pandemic suffered from three fundamental flaws:
Assumptions about respiratory viruses: There was an over-reliance on historical assumptions about how to distinguish between modes of transmission for respiratory viruses. This led to an initial misunderstanding of the level of risk posed by aerosol transmission and a failure to recommend sufficient protections against it.
Insufficient caution: Alternative routes of transmission other than droplet or contact were prematurely ruled out. This meant that pre-existing assumptions that Covid-19 was not ordinarily spread by aerosols became the received wisdom until the contrary was proved.
Lack of structure and expertise of the UK IPC Cell: In reality, the UK IPC Cell acted as the decision-maker as to the content of guidance. However, its remit was insufficiently defined, leading to uncertainty over its role and a lack of accountability. The backgrounds of its experts were also insufficiently broad. These factors created a bias towards maintaining the status quo, rather than responding to emerging evidence dynamically.
1.46	The combined effect of these interconnected flaws led to a failure to properly protect patients and healthcare workers against the risks posed by Covid-19.

We are living in the times of policrises. When disruption of in-person schooling is highly probable. If someone is concerned about ‘learning loss’ but at the same time not advocating for preparation of good robust emergency-remote-education solutions, they are not serious.

A reminder that any analysis of pandemic impacts on kids / education / economy that does not include a serious examination of the effects of #longCOVID is worthless, and one that does not even mention it should be assumed to be deliberately deceptive unless there's a good reason to think otherwise.

Ontario to cut length of teacher education programs nearly in half
Prospective teachers in Ontario will soon have to spend less time in the classroom as the Ford government moves to condense teacher education programs into 12 months.

Please say Airborne. Measles is airborne. It transmits via aerosols. Inexplicit messaging is a form of misinformation. It’s ‘misleading by omission’. #StopMisinformation #StopMeasles #MeaslesIsAirborne

Airborne, ffs. Say. Airborne. Measles doesn't spread easily in some magical locations. It spreads easily where we share air and there is insufficient ventilation and filtration (building, room aka portable HEPA, personal aka CAN95s scale) to prevent inhaling infectious aerosols. Grow up already 🤬

Ran into old friend I'd heard has cancer. She's refusing radiation and chemo because "it's all from the COVID vaccine", and that mistrust now extends to cancer meds. I will NEVER forgive public health for downplaying COVID, leaving the goal wide open for all this garbage.
