Dr Taler’s approach to resistant HTN. Evaluate for pseudoresistance, always consider clues for secondary HTN and selectively test, ALWAYS check aldo/renin possibly during initial visit ( if no MRA or amiloride), Use longer acting meds, add SGLT if CKD or DM, MRA of course, then other meds depending

Despite the availability of safe and effective pharmacotherapy, hypertension
control continues to be a concern. Early results for catheter-based sympa-
thetic RDN have been conflicting. In a meta-analysis including recent RDN
trials, Vukadinović and colleagues showed that RDN vs. sham controls
achieved clinically important and statistically significant lower BP for all
measures, with no safety concerns.
The meta-analysis results cannot be generalized to patients with moderate
to severe chronic kidney disease or isolated systolic hypertension because
they are often excluded from RDN trials. Interpretation of the results may
also be affected by the clinical heterogeneity between the individual trials
and statistical heterogeneity in the meta-analysis. Clinicians should consider
other important but commonly missed treatable causes of uncontrolled
hypertension, such as primary aldosteronism and nonadherence to medica-
tion, before RDN. In addition, responses in individual patients often vary
from mean trial effects, and preprocedural patient characteristics cannot
currently identify response heterogeneity (1). Finally, BP medications are
mostly generic and are inexpensive compared with RDN.
Regulatory approval of RDN in multiple countries allows the use of the pro-
cedure in hypertension that is uncontrolled despite lifestyle changes and
medication use (1), although provider surveys lean toward its use in resist-
ant hypertension, and reducing pill burden seems to be important to
patients when considering RDN (2). Given the heterogeneity of patient
response, shared decision making and tempered efficacy expectations are
important when considering RDN for individual patients with hypertension.
The American Heart Association also emphasized the importance of appro-
priately trained professionals as part of a multidisciplinary team for centers
providing RDN (1).
