Prevention of Tuberculosis in Patients Treated With Biological Therapies: 20 Years' Experience in a Specialized Tuberculosis Clinic in a Low-prevalence Country
This study builds on previous evidence to assess the risk of tuberculosis (TB) in patients treated with biologic therapies (BioT), the role of the interferon-γ-release assay (IGRA) QuantiFERON-TB (QFT) Gold as a stand-alone screening test, and whether periodic re-testing is warranted for patients with a negative pre-BioT screening.MethodsA total of 1368 patients starting BioT were screened for TB infection using 4 screening strategies over 4 consecutive periods: (1) 2-step tuberculin skin test (TST); (2) 2-step TST plus QFT Gold In-Tube; (3) single-step TST plus QFT Gold In-Tube; and (4) QFT Gold In-Tube (or QFT Gold Plus) alone. All patients with TB infection were offered preventive therapy.ResultsTB infection was diagnosed in 327 (23.9%) patients (40.8%, 39.5%, 25.3%, and 14.8% in the first, second, third, and fourth periods, respectively; P = .000). The adjusted odds ratios (ORs) with respect to the first period were 0.89 (95% CI, .55–1.44), 0.49 (95% CI, .33–.73), and 0.23 (95% CI, .15–.36) for the second, third, and fourth periods, respectively. During follow-up, 11 patients (0.8%) developed TB. The probability of remaining TB-free after 11 years of BioT exposure was 99.1%, with no significant differences between screening periods (P = .372). All TB cases in patients with negative baseline screening occurred within the first year of BioT exposure.ConclusionsAlthough BioT-associated TB can be significantly reduced, it is not completely preventable. Neither dual testing nor periodic systematic re-screening for TB infection is warranted after a negative pre-BioT test.

Algorithm for the management of suspected IC with septic shock. *For patients with IC who do not present with septic shock, empiric antifungal therapy should be withheld. In these cases, investigations should focus on obtaining blood cultures and, where relevant, site-directed samples. Biomarkers such as serum BDG may support discontinuation when suspicion decreases but should not be used as a trigger to initiate antifungal therapy. BDG (1 → 3)-β-D-glucan; IC Invasive candidiasis; R ResistanceTable 5: Key actions in management of persistent candidemiaTable 6 Combination strategies for difficult deep-seated refractory infections. 
 Combination therapy is not routine. Reserve it for the specific scenarios above (endocarditis, CNS involvement, ocular disease, renal candidiasis with obstruction, persistent candidemia or very high fungal burden despite source control, suspected/proven resistance, major PK uncertainty or delayed TDM), begin with a pre-defined 48–72 h reassessment, obtain early susceptibility testing, perform TDM for azoles and consider it for echinocandins in complex PK settings, and de-escalate to the narrowest effective monotherapy as soon as feasible. In summary, avoid routine concurrent Amphotericin B–azole combinations for Candida spp. unless there is a compelling salvage reason; prefer alternative pairs (e.g., echinocandin + azole) if combination therapy is needed. If stepping from an azole to Amphotericin B, be aware of potential carry-over effects; if stepping from Amphotericin B to an azole, keep the overlap brief

Nasal bioburden was assessed as follows: 0 = no growth; 1 = few (1-2 isolated colonies); 2 = 1+ (growth in first quadrant only); 3 = 2+ (growth into second quadrant); 4 = 3+ (growth into third quadrant); and 5 = 4+ (growth in all quadrants, high burden).

Text Shot: Bacterial DNA from the oral cavity, respiratory tract, gut, and skin has been detected in atherosclerotic plaques, suggesting a role in chronic inflammation linked to atherosclerosis. Chronic bacterial infections often form biofilms resistant to antibiotics and immune detection, giving rise to a new generation of virulent bacteria in suitable conditions. This study explores the role of the immune system in bacterial‐induced inflammation of atherosclerotic plaques.
