Background
People with intellectual disabilities experience higher rates of
mental disorders, contributing to restrictive practices and
premature mortality. Prevalence data are essential to understanding
the patterns of disease, and for the development of
tailored interventions.
Aims
To systematically examine the burden and pattern of mental
disorders in people with intellectual disabilities across the
lifespan.
Method
We searched six databases (inception to 17 October 2024), and
conducted a manual search up to 15 December 2024, for
systematic reviews on the prevalence of mental disorders in
people with intellectual disabilities with or without neurodevelopmental
conditions. We conducted a narrative synthesis
of prevalence rates, including those stratified by intellectual
disability level, sex, age and autism, where available, and
compared these with published prevalence rates in people
without intellectual disabilities (International Prospective
Register of Systematic Reviews, no. CRD42024610611).
Results
We included 26 systematic reviews: 7 meta-analyses and 19
narrative reviews. Compared with the general population, the
prevalence of schizophrenia (3.55–4.80%), anxiety (5.4–5.5%) and
obsessive–compulsive disorders (2.4%) appeared higher,
whereas that of mood (6–7%), personality and post-traumatic
stress disorders appeared lower. Study quality was moderate to critically low. For syndromic intellectual disabilities we noted
high anxiety rates in fragile-X, Williams and 22q11.2 deletion
syndromes, and in those with co-occurring autism. We found
gaps on dementia, bipolar, substance use and eating disorders,
and limited data stratified by intellectual disability level, sex
and age.

Background
Adults with intellectual disabilities who display behaviours
that challenge (BtC) are more prone to poor health.
Aims
This study seeks to evidence the long-term health outcomes
for those who display BtC.
Method
We conducted a longitudinal cohort study of adults with intellectual
disabilities aged ≥18 years in England, using data fromthe
Clinical Practice Research Datalink Aurum (January 2003 to
December 2023) linked to Hospital Episode Statistics and Office
for National Statistics. Main outcome measures were annual
health checks, general practitioner referrals, emergency visits,
out-patient attendance, in-patient admissions and mortality.
Results
Among 83 166 adults with intellectual disabilities (mean age
38.6 years), 18.5% had a record of BtC, with similar sociodemographic
distributions to those without BtC, but higher rates
of physical and mental health comorbidities and uptake of
annual health checks. A total of 72.5% of participants with BtC
were receiving psychotropic medication(s). Adults with BtC
had higher rates of mental health out-patient attendance
(odds ratio: 1.42, 95% CI 1.33–1.52) and in-patient admissions
(incidence rate ratio (IRR): 1.19, 95% CI 1.09–1.29), but consistently
lower rates of physical health out-patient attendance (IRR = 0.81, 95% CI 0.78–0.84) and in-patient admissions
(IRR = 0.77, 95% CI 0.74–0.79), after adjusting for demographic
and clinical characteristics. BtC were not associated with
mortality after adjustment for comorbidities (hazard ratio: 0.97,
95% CI 0.93–1.00).

tools for symptoms of depression among young adults (18 to 25 years old) in East Asia and the Pacific. A total of
22 studies with 24,069 participants were included, covering both clinical and non-clinical populations. Nine selfadministered
screening tools were identified, with Patient Health Questionnaire-9 (PHQ-9), Patient Health
Questionnaire-2 PHQ-2, and Centre for Epidemiological Studies Depression (CES-D) being the most frequently
studied. PHQ-9 demonstrated moderate to excellent internal consistency reliability, with Cronbach’s alpha
ranging from 0.67 to 0.92, and a pooled AUC of 0.86, indicating strong screening accuracy. PHQ-2, showed an
Area Under the Curve (AUC) of 0.85, high sensitivity (0.96), and moderate specificity (0.80). CES-D exhibited an
AUC of 0.87, good sensitivity (0.81), and specificity (0.78), with high heterogeneity (I² = 74.70 %–86.69 %). The
meta-analysis revealed substantial variability in sensitivity and specificity across settings, with differences in
study methodologies, cutoff scores, and reference standards contributing to high heterogeneity. Additionally,
95.5 % of studies had a moderate risk of bias in patient selection, affecting generalizability. Despite these limitations,
PHQ-9, PHQ-2, and CES-D remain valuable tools for identifying symptoms of depression among young
adults. The accessibility, ease of administration, and strong psychometric properties of these tests support their
continued use in resource-limited settings, though standardization of methodologies and expanded regional
validation are needed to improve screening accuracy and applicability.

Background
Health promotion activities for people with mild cognitive impairment (MCI) may improve their overall health and prevent progression to dementia. We need to understand the best approaches to behaviour change in this population.
Aim
To summarise the types of health promotion interventions and current clinical effectiveness evidence for physical and cognitive functioning outcomes in people with MCI.
Design
State-of-the art review
Method
Medline was searched in April 2025, for systematic reviews synthesising randomised controlled trials testing effectiveness of health promotion interventions in older adults with MCI, published since 2015. Abstracts, titles, and full texts were single screened and, for eligible reviews, the characteristics, intervention type, primary outcome, and evidence for people with MCI, were extracted. Key messages were narratively synthesised across reviews, grouping by type of intervention and outcome measure.
Results
From 1776 titles and abstracts, 57 systematic reviews were eligible. Regular moderate intensity multicomponent physical exercise and memory-focussed cognitive training interventions had the strongest evidence base for improving cognitive functioning. However, combining physical exercise and cognitive training was more beneficial for cognitive domains and physical functioning, in comparison to doing them individually. The evidence for nutrition interventions was mixed. One review of psychological interventions showed improvements for wellbeing.
Conclusion
There is evidence that health promotion interventions can support people with MCI and dementia to improve or maintain their cognitive function, through multicomponent physical activity combined with cognitive training. More evidence is needed for nutritional recommendations, psychological interventions, and social inclusion interventions.
