Randomized trials have unanimously shown the benefits of oral antibiotic treatments for stable patients with bacteremia (1). However, transition to oral therapy has been usually performed after several days of intravenous treatment—frequently up to 7 days (2)—highlighting the need for evidence of the safety of earlier oral treatments (3).

Good timing, I'm creating ID trivia questions currently In the Middle East and Africa drinking camel urine and/or milk is not rare. In a study of 66 pts w cancer who drank a mixture of camel urine and milk, 3 developed infections. Let me know if you want to guess or just want the answer :)

Love the oral choice! I struggle using oral 2nd/3rd gen cephs because folks are used to tiny doses (250-500mg BID) and there limited if any) any PK/PD studies with bigger doses to make up for the 50% bioavailability. We are making big strides w high dose 1st gens (1g Q8-12) w cefadrox and cefalex.

TEMs are definitely in the mix in CO. ESBLs are still more common. We tend/try to use cefepime on TEM phenotypes to save the carbs.

PSA: If you’re ever reversing anticoagulation & someone asks if the patient has had a vasectomy, they’re not being a weirdo. Protamine is made from salmon sperm & post vasectomy patients may have anti sperm antibodies that cross react causing life threatening anaphylaxis!

Fun ID Pharm Q today Pt on mero 2q8 for ESBL infx and has adenovirus viremia. Starting cidofovor + probenecid = mero dose adjustment 1989 study to the rescue! Mero AUC ⬆️ 55%. T1/2 ⬆️ from 0.9 hr to 1.3 hr. ➡️ 2 Q12 x 48 hrs based on probenecid t1/2 of 5-15 hrs pubmed.ncbi.nlm.nih.gov/2808215/
