The issues were created by ChemoCentryx, but Amgen did not take their P4 trial serious! In Europe Avacostar is fully recruited (504 patients). VBDS is not associated with viral infections, I assume, it is a true signal in Japanese

Interesting case, indeed. PR3 in the context of C3G rather uncommon. Undetectable C3 would prompt alternative pathway investigations; also in the consideration cryoglobulins and cryofibrinogen, although C4 reduction would be more common

Phenotypic differences between AAV patients in Japan (compared to USA/EU) are obvious. In this study, we surveyed patients presenting during the early phase of the pandemic. A striking difference in the use of glucocorticoids at 6 months has also been observed! academic.oup.com/rheumatology...

Kidney function recovery becomes a relevant endpoint in crescentic GN, which most of us defined as eGFR increase of 15 ml! We see a modest difference between the groups at 52 weeks in favor of PLEX!

These data are not changing the practice (I would guess). We wanted to give an overview; we are not proposing that people need to be PLEXed or not to be PLEXed based on these results. But we tend to PLEX the younger individuals who will benefit from 2-3 additional ml GFR at w52

No, we defined all analyses a priori. No p-cherry picking as you usually see it. And importantly, these data stem from a clinical trial. Why should an increase of eGFR ≥15 ml/min/1.73 m2 be clinically meaningless. Look at the RaDaR long-term data and risk of ESKD and you will see its importance.

The use of a 15 ml/min/1.73 m2 cut-off to define kidney function recovery was proposed by us and others before and seems to be sensible in AAV. Notably, there was a difference of 8% between groups at week 52 and thus we do believe this represents a clinically meaningful difference.
