Very excited that my and Ian Greener’s new paper (my first, first author pub) is out. Using the Scottish Index of Multiple deprivation, we demonstrate how fsQCA can be a useful addition epidemiologists’ methodological toolkit. jech.bmj.com/content/earl...

"what do we remove?". Curriculums are already jam-packed. I'm not a GP but I'm not clear when these annual reviews would take place. It *might* be of benefit financially if it reduces subsequent GP appointments but it needs be paid for. In the absence of tax rises, what services do you drop to pay

I really like a lot of Shaun's journalism and this is an exceptionally important issue but these solutions feel rather narrow. If you want more education of polypharmacy, you need to eliminate something from the curriculum. The question isn't "should students learn about drug interactions?" but
All medical students must learn the risks of mixing medicines and receive training for how and when to de-prescribe

The NHS should ensure that

anyone over 65 who is considered frail and is taking 10 or more medications is invited for an annual review of their drug

regimen

• 1,850 more geriatricians must be trained by 2030 to ensure there is one doctor for every 500 patients aged 85 and over

How often do we have to debunk this? The moment you apply RCT results beyond the study population (even at a different point in time) you need transportability assumptions again. Observational studies often have the advantage of using actual field data from the population where decisions are made.
