This chart is from @britishelectionstudy.com @jamesdgriffiths.bsky.social, me and Ed Fieldhouse. You can see that 2024 Reform voters mainly came from the Conservatives or UKIP in 2015. This has been a story on the right for *ten years* now, unless something very different happened last week.

Continuity of care and relationship-based GP is vital… not just for primary care - it will create a scaffold with tangible benefit for hospital care too. A point @ics-updates.bsky.social made in our submission to the Palliative Care Commission too GPs deserve a system that reaps their true value

The importance of boundaries for #HCP. Ask yourself: Would refusing to do this cause patient harm, me to lose my job, is it dangerous, or will it damage my own integrity? If the answer is no, hold your boundary, *even if* it inconveniences others, fires up your FOMO, or is career limiting. #acprc25

Move away from blame culture in relation to smoking cessation. Unless you've quit smoking yourself you don't know what it's like to stop, and even if you have you haven't done it from your patient's specific circumstances/situation. Ask non- judgemental questions about smoking status. #acprc25

Havi Carel on breathlessness at #acprc25. We need breath to have a voice, to laugh, to move. To interact with the world. How would you define yourself without these things? Without breath, a person can become invisible - housebound, unheard, isolated.

What does a good clinical conversation look like? I don't know what it *feels* like to live with your condition, or your life; can you tell me? What is worrying you now? As a clinician, you may be expert in a condition, but not in each person's experience. Have epistemic humility. #acprc25

Picking apart pathological versus normal (healthy?) breathlessness. Havi Carel at #acprc25 I think the loss of control is the key here, and everything else spirals from there.

Describing breathlessness- no clear cultural distinction between healthy and pathological in language- "I was so out breath" to describe healthy response to exertion the same as severe pathological breathlessness. Havi Carel at #acprc25

Prof Havi Carel on phenomenology of breathlessness from both a professional and lived experience pov. I like the idea of health as the 'silence of the organs'. When our bodies 'work' we don't pay them attention, it is often only with pathology that our focus turns inward. #acprc25
