Has resistance to #antibiotics in Neisseria gonorrhoeae changed over the past 10 years in #Germany? Researchers from the @rki.de examined temporal trends in antibiotic susceptibility of isolates and possible shifts in sexual networks 👇 bit.ly/EUS3334NG #IDSky #STISky #EpiSky #AMR 🧪🛟
Key public health message

What did you want to address in this study and why?

Gonorrhoea is a sexually transmitted infection (STI) that is common among sexually active men who have sex with men. Antibiotic resistance in Neisseria gonorrhoeae, the bacterium causing gonorrhoea, can make it harder to treat the disease. We wanted to find out whether resistance to antibiotics in Neisseria gonorrhoeae has changed over the past 10 years in Germany and if some groups are more affected than others.

What have we learnt from this study?

We used data collected from laboratories across Germany in a national surveillance system for gonorrhoea to analyse the development of antibiotic resistance. We found that the bacterium is becoming less responsive to the three antibiotics that we examined. This was especially true for samples from males that were not collected from the genital region.

What are the implications of your findings for public health?

Our findings demonstrate the importance of monitoring antibiotic resistance in gonorrhoea. This requires laboratory testing when patients are diagnosed and can help doctors choose appropriate treatments. General STI prevention is important, particularly for men who have sex with men and young heterosexual persons. Easy access to STI testing, timely treatment, safer sex and future vaccination could all help to reduce the spread of gonorrhoea.

Key public health message

What did you want to address in this study and why?
We describe national trends in reported syphilis, gonorrhoea and chlamydia in Poland between 2013 and 2024, with particular attention to the period 2020 to 2024. Poland has some of the lowest notification rates of sexually transmitted diseases (STI) in Europe, making it important to understand whether the increases observed in 2021 to 2024 reflect more infections or improved detection after disruption to healthcare services during the COVID-19 pandemic.

What have we learnt from this study?
Reported notifications of all three infections increased in the period 2021 to 2024, particularly among men aged 20–44 years and in urbanised regions. There were marked geographic and urban–rural differences. While syphilis notification rates approached European averages, gonorrhoea and chlamydia rates remained substantially lower, suggesting that differences in testing intensity and diagnostic access may contribute to the observed disparities.

What are the implications of your findings for public health?
Improving access to STI testing and ensuring that diagnosed infections are consistently reported would help public health authorities to better understand disease trends and target prevention efforts where they are needed most.

Slide with teh Key public health messages of the article:

What did you want to address in this study and why?

West Nile virus (WNV) can cause severe neurological disease in humans. In recent years, France has reported increasing numbers of WNV cases, including in regions with no previous history of virus detection. We therefore wanted to estimate, the prevalence of WNV among blood donors in mainland France, including geographical distribution and risk factors of exposure to WNV, to assess potential implications for transfusion safety and public health.

What have we learnt from this study?

After analysing 44,490 sera from voluntary blood donors collected in 2021–2022, we found a low overall national seroprevalence of WNV (0.97%). However, we also identified signals that WNV had been spreading silently in some unexpected areas, beyond the regions already known to be affected and several associated risk factors including region of residence and blood group.

What are the implications of your findings for public health?

Our findings support the need to extend WNV surveillance in humans, mosquito vectors and other accidental hosts (mainly horses and birds) beyond the known affected areas, including monitoring products of human origin (blood and organs).

Key public health message

What did you want to address in this study and why?

As opposed to traditional reporting, automated data extraction and processing from electronic health records (EHR) can provide more timely and easily accessible data on diseases or infections to inform prevention and control strategies. Here, we describe a European project aimed at using EHR for surveillance of bloodstream infections (BSI) and moving towards comparable data across 17 countries in the EU/EEA.

What have we learnt from this study?

All participating countries reported similar challenges in the implementation of automated or semi-automated EHR-based BSI surveillance systems, including data standardisation, insufficient IT capacity and data protection issues. There is a need in most countries to advance and implement standardised controlled vocabularies, common data models and validation processes.

What are the implications of your findings for public health?

The implementation of EHR-based surveillance systems for BSI will be essential for improving data harmonisation and quality. This project has supported the implementation and strengthening of automated EHR-based BSI surveillance across Europe. The surveillance data can ultimately lead to better infection prevention and control measures for BSI at local, national and EU levels.

Key public health message

What did you want to address in this study?

Infection with hepatitis C (HCV), B (HBV) and D (HDV) virus can lead to a chronic liver disease. We aimed to update estimates of hepatitis C, B and D prevalence in Germany, as previous data were largely based on blood donors or selected groups and therefore may not represent the general population.

What have we learnt from this study?

Hepatitis C, B and D are not widespread in Germany, but they still affect some people. Among the 10,000 people we tested, we found four people with an active HCV infection and 13 people with a past HCV infection. We also found 21 persons with an HBV infection, none of whom were confirmed infected with HDV. Importantly, some people with hepatitis did not know they were infected, showing that not all cases are currently being diagnosed.

What are the implications for public health practice?

Hepatitis C, B and D infections are uncommon in Germany. However, some infections remain undiagnosed, so targeted testing is important. Expanding testing, ensuring that people diagnosed with hepatitis B are also tested for hepatitis D, and improving linkage to care can help close gaps and support the goal of eliminating viral hepatitis.

Visual with teh Key public health message

What did you want to address in this study and why?

Measles is a highly contagious viral disease which can lead to severe consequences. When measles appears in a primary school or daycare, children may have to stay at home for several weeks, which disrupts learning and family life. We investigated whether these long quarantine periods are always necessary. The goal was to stop measles from spreading while keeping the impact on children and parents as small as possible.

What have we learnt from this study?

We found that in many cases, children do not need to at stay home for the full 21 days usually recommended, since shorter quarantine periods of 18 days could be enough. Schools with a vaccination coverage above 70% may also need fewer restrictions. In higher-risk situations, for example when the vaccination coverage is only 20%, vaccinating children quickly after exposure can have an additional benefit.

What are the implications of your findings for public health?

These findings support a more tailored approach instead of general and often strict quarantine rules for all situations. This makes the rules easier to understand, accept and follow. Informed by our simulation results, measles policy in Flanders was revised to reduce the quarantine period from 21 to 18 days in daycares, and in primary schools with a vaccination coverage below 70%.

West Nile virus #WNV is a zoonotic mosquito-borne pathogen increasingly reported in #Europe Is there a way to characterise heterogeneities in the average annual human risk of WNV infection and in WNV surveillance across the region? 🦟 www.eurosurveillance.org/content/10.2... #IDsky #EpiSky #MedSky
Visual summarising the Key public health message of teh papaer:

What did you want to address in this study and why?
West Nile virus (WNV) is transmitted via mosquitoes to humans and other animals. Infections of West Nile virus are increasing in Europe, and the disease has a considerable impact on human health including blood and transplant safety. We aimed to estimate the average annual risk of WNV infection in humans and analyse potential differences in reporting of WNV infections across Europe.

What have we learnt from this study?
We identified hotspots of transmission and learnt that the average annual risk of WNV infection and reporting across the 119 European NUTS 0–3 regions included in this study is heterogeneous. Notably, we found that the locations reporting the largest number of WNV reported cases are not necessarily those experiencing the highest number of infections.

What are the implications of your findings for public health?
This study suggests that WNV surveillance detects different proportions of the actual (unobserved) number of infections across Europe. Seroprevalence surveys can help estimate the true number of infections, the susceptibility of the population and assess the local transmission risk.
