and public health. I “do” home dialysis, first and foremost, and at this point, I am 100% certain that DaVita is where I ought to be, whether the world loves it or not. There are parts of industry that can be grating, but there are other parts that are wonderful. I wish all the best!!

One line is not like the others, right? That line summarizes counties in which none of DaVita, Fresenius, US Renal, American Renal, or DCI operate, yet you find a freestanding dialysis unit not owned by a hospital. What might you hypothesize about the cost of living in such places?

If we set aside editorials and perspective articles... It would be interesting to see what % of RCTs in NEJM and JAMA are industry-sponsored vs NIH-sponsored. I suspect that in those two journals, the former is dominant. JAMA specialty journals are a different topic, for sure.

I use FedEx Office more than any other outfit. I just pick up the poster at the store, so I need not worry about the hotel misplacing it. The price is fine, and more often than not, the location is in the convention center hotel or within a few blocks.

This is one of your best articles. Thank you for putting your experience (really, your life) into words. In reading, I was reminded of my discomfort with the discordance between the claimed "wonders" of home dialysis and the lived experience of patients. Honesty about likely outcomes is important.

That's an interesting point. I've read articles (not in journals) about limited HD capacity in a number of countries. The US is not one of those countries. It'll be interesting to see whether articles in journals provide that context.

I made the same point in a KI Reports paper a few years ago: erythropoietin resistance index (ERI) looks like a novel metric, but really, it provides no better risk discrimination than ESA dose itself. Big ESA dose = higher risk of MACE. journals.lww.com/kidney360/ab...

Clinics have backup supply on hand, and one can always order more. But I’ve long thought it’d be nice if people had a permissible way to inform others in the area of supply that could be shared, whatever the circumstances may be. It’s an idealistic thought, for sure.

I was joking. It’s not an actual market, because regulations prohibit redistribution of supply. The fluid is a prescribed drug, per se, so the dialysis clinics may not reclaim supply. Many people have leftover supply, and other people do sporadically run short of specific colors.
