Le lénacapavir est le premier représentant d'une nouvelle classe d'antirétroviraux: les inhibiteurs de la capside, une protéine qui forme la coque externe du noyau du virus. Il bloque ainsi plusieurs étapes-clés du cycle du VIH en perturbant sa capside, structure essentielle à la protection, au transport et à l'assemblage du virus«Cela constitue une percée historique qui démontre que les outils les plus avancés peuvent être rendus abordables dès le départ», estime Philippe Duneton, directeur exécutif d'Unitaid, une organisation internationale qui finance l'accès à des innovations en santé dans les pays à revenu faible et intermédiaire. C'est cette dernière qui a négocié l'accord avec le fabricant indien Dr. Reddy's aux côtés de la Clinton Health Access Initiative (CHAI) comme cheffe de file technique, ainsi que l'institut de recherche Wits RHI. Parallèlement, la Fondation Gates a conclu un accord distinct avec un autre fabricant indien, Hetero Labs, garantissant ainsi un approvisionnement fiable et la création d'un marché concurrentiel.«C'est un médicament indispensable à avoir à l'heure actuelle, explique Jean Daniel Lelièvre, chef du service d'immunologie clinique et maladies infectieuses de l'hôpital Henri-Mondor, à Créteil (AP-HP), qui développe avec son équipe un candidat-vaccin contre le VIH. Il change vraiment les choses, car il suffit de seulement deux injections par an pour être protégė. » Le principal traitement consistait jusque-là en une pilule à prendre tous les jours, avec tous les risques de stigmatisation et de mauvaise observance du traitement que cela comporte. Sur le terrain, les résultats des études cliniques sont sans appel: l'injection semestrielle de lénacapavir montre une réduction du risque de transmission du VIH de près de 99,9 %.« Avoir un médicament comme le lénacapavir enfin abordable peut être l'opportunité d'avoir un impact sur l'épidémie et de faire baisser le nombre de personnes infectées chaque année», s'enthousiasme Carmen Pérez Casas, responsable principale de la stratégie đ'Unitaid. Depuis 2010, les nouvelles infections ont été réduites de 40 %, mais les données du Programme commun des Nations unies sur le VIH/sida (Onusida) montrent encore 1,3 million de nouvelles infections en 2024.«Ce qui est essentiel avec le lénacapavir, c'est qu'il permet en quelque sorte de fermer le robinet dès maintenant, afin que le problème auquel nous devrons faire face dans cinq à dix ans ne se soit pas aggravé de manière spectaculaire», abonde Trevor Mundel, président de la santé mondiale à la Fondation Gates.

Le ministère a indiqué que la propagation du virus restait principalement concentrée en République démocratique du Congo et en Ouganda. Il a ajouté que l' Organisation mondiale de la santé « continue d'évaluer le risque global pour la santé publique comme étant faible ».
