AI is fundamentally changing the internet, shifting from user-driven search to AI agents providing direct answers, raising concerns about transparency and user agency. This transition could make the web more controlled & less open, AI agents may obscure commercial influences or platform incentives.

Wachter and I agree on an essential point. Given a curated set of facts, AI will outperform humans in diagnosis and management, and doctors may sometimes get in the way. Where we disagree is what the physician still adds once AI can act on its own with patients.

AI is going to get better and better. When it comes into a field, it’s clunky, then improves, briefly loses to human-AI hybrids, then surpasses them. Medicine is next. I laid that all out in my recent @jama.com paper with @vkhosla.bsky.social, Neal Khosla, and Abe Baker-Butler.
