Nice study and the big ticket result for me is the effects on eGFR slope of Sibeprenlimab compared to others. Would be interesting to add in dual inhibitors like Atacicept for comparison!

A patient with type 1 diabetes received genetically modified allogeneic islets and no immunosuppression. Results obtained through 14 months support β-cell function and a lack of immune response against the allograft. Full study results: nej.md/4wW5wGB

Also I take the point about the Obi v Ritux trial needing to be big to show difference but maybe nephrology needs to be braver. Yes it would be hard and expensive but many other specialties have been doing these head to head marginal gain trials for years e.g. haem, onc, cardiology

Disappointing that, while FDA has made big strides over last 5 years in licensing for kidney disease, this kind of playing of the system is required. My wife is a hepatologist and is still astounded by the Terlipressin situation in US when in UK we have been giving on ward for years.

Never been convinced by PLEX. PEXIVAS really only showed small benefit at 12 months at the expense of big additional risk. Mechanistically even in pulm haemorrhage it has never made sense to me - we’re doing a treatment that effectively anti coagulates someone who is bleeding??

Definitely because it’s a pain in someone who is proteinuric but also has HF - you can never get to the max possible dose of valsartan with the combination which is what I really want in those folks.

True… Though could be done if funded from government agencies/charities etc. As for voclosporin - used it once and never again! Product info mandates monthly bloods for entire time they’re on it. Cheap as chips generic Tac all the way for me 😀

A head to head comparison between Obi and Ritux would be v helpful. Theoretically Obi should give better B cell clearance and more durable response- might be like in lupus, why bother with Ritux when Obi works better?

@askrenal.bsky.social anyone in England/Scotland/Wales - do you have to dispense Kinpeygo and Sparsentan direct from hospital pharmacy or is it available via community pharmacy? In NI we have to dispense direct from hospital due to an issue about cost parity in primary care.

Our unit has been routinely doing HDF for a number of years now. Great to see accumulating evidence of benefit. One practical point we learned though… Our unit was designed in 1990s for HD. The drains didn’t cope well with the turbid effluent from HDF so the place stank until we got them fixed!

Thanks - has bone marrow/splenic involvement too so low WCC chronically making Aza a no-go. Saw @kidneyboy.bsky.social’s post about lack of discussion. Wonder if it’s nothing to do with platform but that attitudes to social media have changed after twitter became a s**tshow?

#askrenal @nephjc.bsky.social have a young male with multisystem sarcoid. Eyes, liver, hilar LNs and interstitial nephritis. Granulomas so big you could pick them out without magnification on histo slide! Any experience of anti TNF as steroid sparing for renal involvement?
