JAMA study: Not routinely assessing gastric residual volume (GRV) in 4460 critically ill children was noninferior to usual care (survival 25 days, OR 0.95), increasing nutritional achievement at 72 hrs. Both groups had median 25 days survival/vent-free days.

At lower saturations, this translated to greater over-estimation of SaO2 (consistent with historical papers). At higher saturations, it translated to less under-estimation (as seen in the preprint, where SaO2s were generally high).

I think our new study actually bridges the gap between the previous evidence and the FDA-funded preprint. The consistent message we found was that the devices, on average, gave higher readings on darker skin tones.

I completely agree that if this was entirely driven by a reduction in vasopressors then this is more of a process measure than a patient centred outcome, but the supplement suggests ventilation and RRT were also reduced

If we design trials to detect superiority based on P<0.05, should we then throw them out as not sufficiently convincing because P=0.04? I’d be more concerned whether this is showing the benefit of a complex CRT protocol or a clinician at the bedside.

I don’t think it was site selection, although some of our best recruiting sites were maybe in somewhat less diverse parts of the country. Interesting to reflect on whether there was any lack of equipoise.

Interesting point, Jack. If it reflected the UK population of ventilated patients (see supplement) we’d have expected more like 500 black patients so yes, somewhat under-represented but we would not have anticipated large numbers.

5. Results should be offered in a way that is accessible to participants, both in terms of the communication mechanism and the content.

It is absolutely correct that the adjusted analysis is the primary analysis and, provided it was fully pre-specified, it is not “torturing the data”.

I’m afraid the error here is presenting the unadjusted results. I’m not a big fan of the stepped wedge design, but if you use it then it must be adjusted for clusters and time.

An important clarification here: the P=0.02 on mortality was a test of non-inferiority, i.e. the difference was significantly below the non-inferiority margin. It does NOT mean that mortality was significantly higher in the PCT group.
