Since I started seeing patients presenting with RTIs, I have had about 4 chest infections over the winter period, despite wiping things down and wearing masks. But I admit not as often as I should have.

Very enjoyable consultation by explaining: - pathology of pain and condition not using medical jargon - current guidelines and caution with regular opioids - Importance of non-pharmacological options like CBT The most challenging part was managing the patient's expectations.

I always feel privileged when a GP asks me to have a joint consultation to help with a patient with complex conditions. Young patient in his 20s with FM and mixed depression/anxiety. Drug therapy choices limited because of intolerance and side-effects.

The new BTS/NICE/SIGN asthma guideline is out, recommending a SABA-free pathway. Asthma is a chronic airway disease. Any treatment should include drugs to reduce the underlying inflammation. Salbutamol has no anti-inflammatory properties. So why use it? 2 main emerging pathways: AIR and SMART

- Stay away from oral steroids - inhalers not indicated - no need for extended abx course in LRTI #AMR (unlike in #bronchiectasis) - MDT working, better communication primary/secondary care

Take away points for ILD not associated with other respiratory diseases: - ILD is NOT COPD - have ILD as a differential if #SOB, ongoing #cough, quick desaturation - earlier diagnosis lead to better outcomes: CXT, HRCT, PFT, LTOT, PR - crackles: once you've heard them, you'll remember them.
