Open Access: Need to Assess On-Demand Versus Continuous Treatment in Allergic Rhinitis in ARIA-EAACI Guidelines. Corresponding author: Jean Bousquet Read the article here: doi.org/10.1111/all.70498
There is ongoing discussion on whether patients with allergic rhinitis should receive continuous long-term or on-demand treatment. In this Letter, Bousquet et al., point to the low medication adherence observed in MASK-air studies and by physicians with allergic rhinitis. The study recognizes that this data was obtained from selected populations and that there is a need for dedicated guideline recommendations.

The ɑ-hairpinin of Ara h 1 contains a dominant epitope that is both frequently recognized and blocks a majority of the IgE against this domain. Including this region in tests may improve diagnosis of peanut allergy. The trimeric structure of the vicilin region explains the ability of Ara h 1 to induce anaphylaxis with a single antibody.

This study shows that sodium dodecyl sulfate (SDS) and cocamidopropyl betaine (CAPB) directly impair esophageal epithelial barrier integrity in primary eosinophilic esophagitis (EoE)-derived models. SDS induces stronger inflammatory, oxidative stress, and keratinization responses than CAPB. Both surfactants promote epithelial remodeling and partial epithelial-mesenchymal transition (EMT), with CAPB showing a distinct extracellular matrix remodeling profile. Common surfactants in personal care products may act as environmental aggravating factors in eosinophilic esophagitis.

Stevens-Johnson syndrome/toxic epidermal necrolysis (SJS/TEN) is a rare, albeit life-threatening, adverse skin reaction to certain medications. Kinoshita et al. demonstrate that neutrophil extracellular trap (NET) degradation is impaired in SJS/TEN patients due to DNase 1 dysfunction, as patients were observed to exhibit significantly reduced DNase 1 activity and levels. The NET accumulation is further aggravated by inhibition of DNase 1 by G-actin released from necroptotic keratinocytes.

In this new study, SIAF researchers and their international collaborators investigated soy lecithin and DATEM, two widely used emulsifiers found in processed foods, including some peanut butters.


Using human gut-on-a-chip and adult stem cell-derived colon organoid-on-a-chip models, together with human immune cells and mouse models, the researchers found that both emulsifiers:
🔹 Disrupted intestinal epithelial barrier integrity
🔹 Activated pro-inflammatory pathways, including TNF and NF-κB signalling
🔹 Induced local and systemic inflammatory responses
🔹 Stimulated particularly IgEantibody responses 

Soy lecithin additionally induced oxidative stress and increased IgE levels in mice, while DATEM enhanced IL-4-induced IgE production in human peripheral blood mononuclear cells.

These findings support the Epithelial Barrier Theory and highlight the need to consider barrier dysfunction and low-grade immune activation when evaluating the biological effects of commonly used food additives. Further studies are needed to determine their long-term effects in humans.

Using TLR4 knockout bone marrow-derived macrophages and HEK293 cells transfected with TLR4, Zlatanova et al., demonstrate that the enzymatic function of Act d 1 is essential for TLR4 activation. The authors propose an alternative mechanism of receptor activation where Act d 1 cleaves the TLR4 ectodomain, thereby removing the domain responsible for maintaining TLR4 in an inactive state in the absence of its ligand.

This is the first randomised placebo-controlled trial of intralymphatic #immunotherapy using a microcrystalline tyrosine-adjuvanted grass pollen allergoid. Three ultrasound-guided intralymphatic immunotherapy (ILIT) injections were well tolerated and reduced combined symptom-medication scores compared with placebo. The findings support further optimisation and clinical development of allergoid-based ILIT for #grasspollen allergy.

Both Full and Limited FESS result in clinically meaningful relief of disease-specific health-related quality of life (HRQoL) and nasal breathing, effectively improving nasal symptoms and sleep disturbance in chronic rhinosinusitis with nasal polyps (#CRSwNP) patients. Clinically meaningful differences in decreases of SNOT-22 scores (22-item Sino-nasal Outcome Test) between the groups were not found at 12- and 24–month follow-up. Differences in improvement of anosmia, and decreases in polyp load were seen between the groups.

This study aimed to determine whether thymic stromal lymphopoietin (TSLP) induces a pathogenic immunometabolic program in human type 2 conventional dendritic cells (cDC2s) and whether tezepelumab can directly reverse this process. TSLP rewires human cDC2 metabolism by increasing both glycolysis and mitochondrial oxidative phosphorylation. This metabolic shift drives a pro-type 2 phenotype required to generate pathogenic Th2 cells and functionally impaired FOXP3+ Tregs. Tezepelumab effectively restores normal metabolic and functional properties in cDC2s, inhibiting Th2 polarization and rescuing Treg function.

This study aimed to identify a set of serum biomarkers that robustly distinguish prespecified severe from mild atopic dermatitis groups. Serum proteomic profiling distinguishes severe from mild atopic dermatitis, revealing an epithelial-enriched severity footprint linked to lactate dehydrogenase (LDH)-associated tissue injury and Th2/Th22 inflammation. Machine learning identified a 9-protein biomarker signature that discriminated severity groups with high cross-validation accuracy (AUC = 0.989).

This study evaluates the effectiveness and safety of #dupilumab, an IL-4Rα inhibitor, for the treatment of cholinergic urticaria (ChoIU), a rare condition manifested as pruritic wheals triggered by sweating and increases in body temperature. In a phase 2 randomized, double-blind trial of 42 adults with ChoIU, no significant difference was observed between patients treated with dupilumab and those treated with placebo at weeks 16 and 32.

Atopic dermatitis (AD) is characterized by microbial dysbiosis. This study aimed to evaluate the effects of abrocitinib, a JAK1-selective inhibitor, on the skin microbiome and its potential associations with clinical outcomes in patients with AD. Data from 34 patients with severe chronic rhinosinusitis with nasal polyps (CRSwNP) were collected retrospectively from 6 European study centers. Patients showed high type-2 comorbidity burden, supporting dual therapy as a potential option in complex type-2 diseases. Significant improvements were observed in SNOT-22, olfactory function, and nasal polyp score after 6 months compared to baseline with single biologic therapy.

This study aimed to identify metabolic subgroups (metabotypes) of adult #asthma using plasma metabolomics, with potential implications for personalized treatment. Four metabotypes with distinct lipid-class signatures were identified. Group 1 had elevated ether-linked phosphatidylcholines with young, female-predominant early onset; Group 2 showed elevated lyso-phosphatidylcholines and altered amino acid metabolism; Group 3 exhibited reduced sphingomyelins and phosphatidylcholines in middle-aged, non-obese patients; and Group 4 demonstrated elevated triacylglycerols and diacylglycerols with the highest body mass index.

Gene expression of Ara h 2-specific B cells over the course of the peanut oral #immunotherapy (OIT). Isotype of allergen-specific B cells changes through OIT towards IgG4. Peanut OIT changes the gene expression of allergen-specific memory B cells. OIT patients showed increased gene expression for regulatory B cells and immune regulation. Desensitized patients had upregulated antiviral genes compared to tolerant patients.

Single-cell transcriptomic, T-cell receptor, and spatial analyses distinguished mycosis fungoides-like atopic dermatitis (mfAD) from classic mycosis fungoides (MF) and atopic dermatitis (AD). mfAD showed oligoclonal T-cell expansion with chromosomal stability, enriched CD4/CD8 tissue-resident memory T cells, and a dominant Th22/IL-13 inflammatory signature. The findings clarify mechanisms of #dupilumab refractoriness and highlight JAK inhibition as a potential therapeutic strategy in mfAD.

Soy lecithin (E322) and diacetyl tartaric acid ester of mono- and diglycerides (DATEM, E472e), the two commonly used food emulsifiers introduced into many foods including peanut butter and puffed bread, in the 2000s, impair intestinal epithelial barrier integrity in human gut-on-a-chip and induced pluripotent stem cell colon organoid models. Both emulsifiers activate NF-κB/TNF-driven inflammatory pathways and induce stress responses and apoptosis in epithelial cells, with soy lecithin additionally triggering oxidative stress and suppressing aryl hydrocarbon signaling in gut epithelial cells. This study uses human nasal epithelial cells and BALB/c mice to evaluate the role and molecular mechanism of ferroptosis in house dust mite (HDM)-induced allergic #rhinitis (AR), focusing on ferroptosis-related responses and nasal #epithelialbarrier disruption. HDM exposure induces lipid peroxidation-driven ferroptosis and epithelial barrier disruption. GPX4 protects against this damage, whereas GAS1 promotes ferroptosis by suppressing GPX4, a mechanism validated in GAS1-knockdown mice.

Young children (0–9 years) and residents of urban areas appeared particularly vulnerable to respiratory allergy development and exacerbation. Incidence showed nonlinear associations with particular matter 10 (PM10) PM2.5, temperature, and alder/birch pollen, while dispensed prescribed medication rate showed similar patterns, except for NO2 and the interaction O3*NO2. Combined reductions in PM10 exposure and alder pollen may have the greatest potential to reduce respiratory allergy burden.

Experimental EoG recapitulates histopathological features of human disease, including foveolar elongation, fibrosis, eosinophilia, mastocytosis and shared transcriptional programs. Eosinophilia and mastocytosis were regulated by IL-4Rα, whereas mucosal thickening and foveolar elongation were regulated by IL-4Rα and IL-13Rα1.

In bronchial biopsies, senescence-associated secretory phenotype (SASP) and p53 pathway enrichment scores are elevated in severe asthma versus non-severe asthma and healthy controls. SASP enrichment is validated in the independent Newcastle severe asthma cohort (NOVA). SASP enrichment is also elevated in nasal brushings of participants with nasal polyps, independently of asthma status. Senescence pathway enrichment associates with exacerbations, daily oral corticosteroid (OCS), lower post-bronchodilator (post-BD) forced expiratory volume in 1 s (FEV1%) and macrophage/oxidative phosphorylation (OXPHOS) signatures, but not with Th2/eosinophil signatures, supporting a non-T2 mechanism.

Machine-learning analysis of 502 well-characterised food-allergic patients was used to identify key determinants of poor food allergy-health-related quality of life (FA-HRQL), defined as FAQLQ ≥ 5. The most relevant predictors were adult age, female sex, multiple food allergies, severe reactions, need for urgent care, onset before 18 years, and longer disease duration. Adult women with multiple food allergies showed the poorest FA-HRQL.

Cobalt chloride (CoCl2) and cobalt nanoparticles (CoNPs) induce a shared, severity-dependent inflammatory response in cobalt-sensitized skin characterized by activation of interferon signaling, inflammatory pathways (TNF/NF-κβ, IL6-STAT3), apoptosis/unfolded protein response (UPR), cell cycle programs, and glycolysis. Despite comparable nominal exposure, the two cobalt forms exhibit distinct tissue distribution: CoCl2 penetrates deeply and accumulates intracellularly across epidermal and dermal compartments, whereas CoNPs are largely retained in the stratum corneum. Direct comparison reveals divergent transcriptional programs, with CoCl2 preferentially inducing interferon and antiviral responses (e.g., IRF7), and CoNPs promoting keratinization and epithelial differentiation (e.g., KRT25). Together, these findings demonstrate that cobalt speciation and penetration depth shape tissue exposure and downstream immune responses in allergic contact dermatitis.

AI-assisted readouts provide wheal measurement suggestions, thereby reducing inter- and intra-physician variability. This study analysed real-world SPAT data from 37 hospitals across 5 European countries, including 10,756 patients with various allergic diseases. Their findings support the integration of an AI-assisted readout model in clinical practice.

Restoration of CPT1A expression improved epithelial barrier integrity via increasing mitochondrial respiration and ATP production in airway epithelial cells. Restoration of deficient epithelial CPT1A may represent a new treatment approach for severe asthma.
