I don't think it's groupthink or lack of diversity; I think it's the hierarchical mindset of the decision makers that's the ultimate issue here. 1. The Boss is always right. 2. When The Boss is wrong, see #1. 3. Make The Boss look good = promoted; make The Boss look bad = fired.

Groupthink. Lack of diversity in decision making--age, gender, race, socioeconomic background. With groups esp men who are v similar, you get a high-risk frat boy mentality happening. We are seeing it in real time.

Just about every technology disaster you can name - Chernobyl, the Challenger, etc. - had people close to the workings who spotted potential problems and escalated them, only to be over-ruled by managers thinking only about budgets and deadlines.

The level of media literacy required to wade through #WNBA coverage is criminally high. If you don’t watch games yourself you end up experiencing a completely parallel reality. And this is the New York Times but social media - especially Twitter and Instagram- don’t mitigate things.
