Effect of exercise therapy and self-management support on multimorbidity: Secondary analysis of the MOBILIZE trial - Communications Medicine
Bricca et al. present secondary analysis of the MOBILIZE trial examining whether adding a 12-week exercise and self-management program to usual care benefits people with multiple long-term conditions. The program lowers systolic blood pressure compared to usual care but does not significantly change other biomarkers.

Increases in MVPA of 5 min/day might prevent up to 6% of all deaths in a high-risk approach and 10% of all deaths in population-based approach. Reducing sedentary time by 30 min/day might prevent a smaller, but still meaningful, proportion of deaths. www.thelancet.com/journals/lan...

7/10 So what does this mean? A personalized mix of exercise therapy and self-management support might help people with multiple chronic conditions feel better—without adding risk.

Alessio Bricca @abricca.bsky.social · 30/06/2025
6/10 Importantly, there were no major safety concerns. Both groups had similar numbers of serious health events—so the program didn’t increase risk. ✅

5/10 📈 The results? People in the program group had a statistically significant improvement in HRQoL of life compared to the usual care group. They also rated their own health more positively. Other outcomes like physical function and mental health were similar across groups.

4/10 Participants were randomly assigned to one of two groups: 🔹 Usual care alone 🔹 Usual care + 12-week program of personalized exercise and support for managing health. The development of the 12-week program can be found here.

In adults with at least one chronic disease, anti-inflammatory diets lead to small improvements in physical component HRQOL, which may not be clinically relevant. No effect was found on the mental component or general HRQOL. Further high-quality RCTs may change this conclusion.
