The letter reads in part:

Thank you for utilizing John Muir Medical Center. Our office has billed your insurance carrier, ANTHEM BLUE CROSS PPO - ANTHEM BLUE CROSS PPO,, for the services provided on/during 07/15/26 to 07/20/26.

We were recently notified by your insurance carrier that the claim is either pending or has been denied as they are requesting information from the patient/guarantor. The information being requested is usually in reference to either coordination of benefits or accident information.
Blue Cross has indicated that they cannot complete/approve our claim for payment claim until the information
requested from you is received.
At this time, we ask that you contact your insurance carrier directly in order to avoid a claim denial.
