in addition to being a big creepy doofus this guy also spent the 90's pushing for the regulatory reform that cleared the way for the 2008 financial crisis so it is really unclear what enlightened ideas this guy really needs to be teaching. but those ideas are probably really valuable to some donors

funnily enough the "blameless postmortem" tries to emulate this in software system outages but the unrelenting demands of capital tend to cool managerial jets on the process. or even more quickly when the process refers to "the outage mgmt was warned about when destaffing critical team, happened".
