The driver - to reduce OP waiting lists might be achieved but at what cost to the patient? Given the comments from other GPs on other platforms there's a sense that the referral process is perhaps being weaponised, gamed and/or embroiled in bureaucracy. Would welcome a conversation, Helen.

If this is messed up AGAIN, confidence in NHS will be lost regardless. Perhaps time to get some different, silo-agnostic voices around the table rather than the usual suspects? Thoughts?

Agree, but there has to be a viable alternative ready to take its place, not an FDP Mk2 but a more flexible & intelligent approach, one capable of measuring, monitoring & addressing the core problems incl clinical, flow, delays, capacity, bottlenecks, both within & beyond the NHS boundary.

interesting post from @andrewlearmonth So why not a different approach that's silo agnostic & vendor neutral? An approach that's more, flexible & intelligent? One where primary & secondary care share the same data repositories but in different ways, with real access control across all geographies

Agree, provided that it is the clinical record - interventions, obs, diagnosis, plans, examinations but needs to be vendor neutral, perhaps curated by clinicians, and reach across all boundaries where the above occurs. Access to such records could be managed in a far more intelligent way too.

In answer to your Q: Erm NO but its not just clinical IT either. Back in 2018 we identified 16m hrs lost to unnecessary manual overheads across Health & Care, due to poor or missing tech, theBMJ found a further 13.5hr All told its probably closer 45-50m hrs p.a. but happy to stand corrected.

Indeed. It's shameful Things could be very different today - Everyone would have full visibility of the patient journey as a whole, know where the REAL issues reside, allowing for far better coordination. Focus, time, money & effort in the right places, with +-impact visible in real-time.

Excellent rant. Understanding the size of long waits is one thing but understanding the broader WHYs + how to resolve is something that ought to be the focus. The success of which will be observed in a reduction in 12 hour waits. It's at this point many reach for a single reason where in reality

Internal coordination failures for sure but also coordination for repats, rehab, mental health and other services, where the tech is missing and/or so incredibly poor that colleagues are saddled with the manual overhead burden and ropey spreadsheet solutions in order to hide the problem.

We know, we know... Far superior, elegant & cost effective approaches than this we're available as far back as 2018. Whilst this may provide a nice shiny front-door to lure users, they'll still be tethered same old tired systems from the same old vendors clunking away in the background.
