Pennsylvania has now reported 4 measles-associated deaths in 2026, its first measles-associated deaths in 35 years. One person was just 18, unvaccinated, and died from ADEM, a rare, severe neurological complication. Measles isn’t “just a rash.” MMR vaccination is 97% effective.

The U.S. now has more measles cases than we’ve seen in 35+ years. Measles didn’t suddenly become harder to prevent. What changed is our relationship with vaccines, public health and expertise. Diseases we once had largely under control can come back.

We always get excited about the advances in medicine but there’s less energy for the specific policy changes that would make these advances available to all who need them. Medicare for All or some other form of universal health care is so important to end disparities in access to novel treatments.

It’s profoundly devastating that Jason Arday is no longer here with us. I hope he’s with the ancestors and they’re enveloping him in the love, warmth, and tenderness he deserved. Whatever questions may have existed about his work, nothing justified the cruelty and dehumanization he endured.

Watching AOC document her egg-freezing experience brought me back to medical school. We learned about fertility declining with age, but egg freezing was never presented as a realistic option. We were given the countdown, but not the conversation.

The measles outbreak we’re seeing today didn’t start this year. It reflects years of declining vaccination rates and weakening community protection. Public health is often invisible when it works. When prevention starts to fail, the consequences become impossible to ignore.
