Shematologist, MD @shematologist.medsky.social · 30/09/2026The goal is to treat the underlying cause so they eventually don’t need supplementation but that’s not possible for everyone 010
Shematologist, MD @shematologist.medsky.social · 30/09/2026Yes treatment guidelines to follow. Ferritin target is 30 in most adults, 50 if symptoms or risk factors, 100 if inflammation 011

Shematologist, MD @shematologist.medsky.social · 25/09/2026Ugh that’s rough. IV might be a good option and these are for optimal absorption but there’s still some absorption if you do it differently 120
Shematologist, MD @shematologist.medsky.social · 25/09/2026I’m apparently now on TikTok posting about iron 🤦♀️ 5292
Shematologist, MD @shematologist.medsky.social · 24/09/2026The nicest thing about it is being able to chat with the patient without taking notes or staring at the computer 220
Shematologist, MD @shematologist.medsky.social · 22/09/2026The guidelines are meant to be used internationally and my good friend Michelle Sholzberg already changed the majority of labs in Ontario! 110
Shematologist, MD @shematologist.medsky.social · 21/09/2026I’m so happy that the @ash.hematology.org guidelines are getting a lot of attention but all these influencers who don’t actually understand iron deficiency posting about it is making me CRAZY. 6555
Shematologist, MD @shematologist.medsky.social · 18/09/2026It’s been a long time coming but hopefully will help many people. It’s hard with inflammation so tsat can be helpful (needs to be fasting) and if <20% is diagnostic 020
Shematologist, MD @shematologist.medsky.social · 18/09/2026Yes I would consider autoimmune disease as inflammatory 110
Shematologist, MD @shematologist.medsky.social · 17/09/2026pubmed.ncbi.nlm.nih.gov/34329578/pubmed.ncbi.nlm.nih.govPhysiologically based serum ferritin thresholds for iron deficiency in children and non-pregnant women: a US National Health and Nutrition Examination Surveys (NHANES) serial cross-sectional study - PubMedNone. 110
Shematologist, MD @shematologist.medsky.social · 17/09/2026Yep, less data and some data on other parameters having an inflection point around 20 in kids 110
Shematologist, MD @shematologist.medsky.social · 17/09/2026If menstruating I would use the 30 cutoff. There’s very little data in middle child ages as iron deficiency very rare outside of those who menstruate 021
Shematologist, MD @shematologist.medsky.social · 17/09/2026So common in donors! It’s a real problem. 040

Reposted by Shematologist, MDLissa Harris 🌈🗑️🔥⚔️🐀 @lissaharris.bsky.social · 17/09/2026THANK you my ferritin was in the toilet a few years ago - I think it was 14 when I realized I needed to test for it. never showed up in my hemoglobin numbers, but iron deficiency made me feel like I was dying very very slowly I fixed it with a hysterectomy tl;dr low ferritin w/o anemia is awful 1225

Shematologist, MD @shematologist.medsky.social · 17/09/2026No, if low and symptomatic then need treatment regardless of hemoglobin 170
Reposted by Shematologist, MDNatalie (prev “NanouTheNomad”) @drnataliestahl.bsky.social · 17/09/2026Our lab’s lower limit of normal for ferritin for adult women is um much much much lower than this 1181
Shematologist, MD @shematologist.medsky.social · 17/09/2026These numbers are meant for otherwise healthy adults. In those with inflammation cutoffs should be even higher 021
Shematologist, MD @shematologist.medsky.social · 17/09/2026Thank you for all of that important work 💜 020

Reposted by Shematologist, MDdr dr liebs 🩷💜💙 @liebsmachine.bsky.social · 16/09/2026On a personal note, my lowest ferritin was 9 (after a few, in retrospect, ill-advised RBC donations) and has only made it up to 26 after several months of PO supplementation. Very relieved insurance can’t give me/my PCP pushback on infusions with these new guidelines. AFAB folks take note!! 0121
