10 years today, after a David vs Goliath FOI legal battle that damaged Alem Mathees, White et al were forced to release data from PACETrial (that cost UK taxpayers £5M) showing graded exercise therapy & CBT didn’t lead to an increased recovery rate for CFS. Abstract

Background: The PACE trial was a well-powered randomised trial designed to examine the efficacy of graded exercise therapy (GET) and cognitive behavioural therapy (CBT) for chronic fatigue syndrome. Reports concluded that both treatments were moderately effective, each leading to recovery in over a fifth of patients. However, the reported analyses did not consistently follow the procedures set out in the published protocol, and it is unclear whether the conclusions are fully justified by the evidence.

Methods: Here, we present results based on the original protocol-specified procedures. Data from a recent Freedom of Information request enabled us to closely approximate these procedures. We also evaluate the conclusions from the trial as a whole.

Five-time Olympic gold medallist Kaylee McKeown will be sorely missed when Australian swimmers take to the pool for the start of the Commonwealth Games in Glasgow on Friday.
But her decision to withdraw from the competition due to glandular fever is a smart move for her long-term health, experts say, and represents a shift in perception of the risks it can, in some cases, develop into a lifelong battle with chronic fatigue syndrome.
The seriousness of glandular fever, with its benign-sounding nicknames of “mono(nucleosis)” or the “kissing disease”, is too often underplayed given its potential without careful management to become chronic fatigue syndrome, an illness that at its worst can leave people unable to work, or even bedridden.
This is the view of University of Melbourne biochemist and medical researcher Chris Armstrong, who specialises in the chronic outcomes of infectious disease.
“Kaylee has made a smart decision. There are real dangers to anyone’s long-term health if they try to push through a lingering infection like this – particularly when sleep has stopped being refreshing, fatigue persists and symptoms worsen after exertion,” Dr Armstrong says.
“I hope her decision is a signal that there is a changing perception to the risks of pushing through a lingering infection. Resting properly is one of the few things we know can avoid a longer-term fatigue syndrome.”
McKeown withdrew from the Commonwealth Games after competing unwell in the Australian selection trials and failing to recover.
