Why Microdosing in Normal-Weight Patients Remains Unproven

Despite the mechanistic plausibility, several critical gaps prevent recommending this approach:

No dedicated trials exist. There are no completed or registered clinical trials evaluating sub-weight-loss doses of any GLP-1 RA specifically for anti-inflammatory endpoints in normal-weight individuals.

The "minimum effective anti-inflammatory dose" is unknown. While preclinical data suggest anti-inflammatory effects at sub-anorectic doses, the threshold for clinically meaningful inflammation reduction in humans has not been established. The dose-response curve for inflammation may differ substantially from that for weight loss, but this has not been characterized.

Even low doses cause GI side effects. Nausea, vomiting, and appetite suppression occur even during the initial titration phases (e.g., semaglutide 0.25 mg), which could still cause unwanted weight loss in a normal-weight person.

Risks may not scale linearly with dose. While some adverse effects (GI symptoms, gallbladder disease) are dose-dependent, others — such as the potential thyroid cancer signal — may be related to duration of GLP-1 receptor activation rather than dose per se, meaning even low doses over long periods could carry risk.

And the risk of being underweight and malnourished should not be underestimated.

Gallbladder disease: A 37–46% increased, dose-dependent risk without sufficient metabolic benefits to offset biliary complications.
Thyroid/cancer signals: Possible increased thyroid cancer risk with long-term use, without obesity-related cancer risk reduction to balance the concern.
Attenuated cardiovascular benefit: Major adverse cardiovascular event reduction is not significant at a BMI of 25, meaning risks may be incurred without proportional improvement in hard cardiovascular outcomes.

Do you mean for the cardiovascular and anti-inflammatory benefits? I asked OpenEvidence because this is not my area of expertise. Benefits are less for normal weight individuals.

Yep. In Kansas one of the Medicaid programs will no longer cover any of the services at our children's hospital or any of the pediatric clinics affiliated with them (I think all area groups are now affiliated).

Most people focus on the spread of hepatitis B through sex and injection drug use, but anyone can get hepatitis B, including children. Before hepatitis B vaccine was recommended for all children, each year about 18,000 infants and children were infected with the virus. About half of those children were infected during delivery or shortly thereafter because their mothers were infected. The other 9,000 often never knew when, where or how they were exposed to the virus. Three features contribute to hepatitis B’s stealthiness:

Invisible blood
While hepatitis B replicates in the liver, the virus, as well as proteins from the virus, circulate in the blood of infected individuals. This is how it is spread from person to person. Some people infected with hepatitis B virus have about 10 million to 1 billion hepatitis B virus particles in every milliliter of blood (a milliliter is about one-fifth of a teaspoon). But even among those with lower quantities (e.g., 100 to 1,000 virus particles per milliliter of blood), hepatitis B can be transmitted in amounts of blood not visible to the naked eye. Some body fluids have also been demonstrated to spread the virus, likely because of undetectable quantities of blood, including saliva, semen and vaginal fluids. 

The presence of the virus in small or invisible amounts of blood means transmission can occur in unexpected ways or at places that people might not think about. For example: 

Although hepatitis B virus DNA and hepatitis B virus surface protein can be found in breast milk from mothers who are hepatitis B positive, a baby won’t be infected by the milk. However, if that mother has cracked or bleeding skin or nipples, the baby can be exposed to the virus while breastfeeding. In fact, the recommendation is that in this situation a hepatitis B-positive mother should not breastfeed until healed because of this likelihood.
Hepatitis B can also be spread by other household contacts as well as people in day cares or schools and other non-household contacts. Examples include contact with blood from cuts in skin; pre-chewing of food; sharing gum or other foods; sharing toothbrushes, razors, utensils or other items that might contain undetectable amounts of blood, like washcloths or clothing; and contact with blood during sports or other physical activities.a final screen shot: Hepatitis B vaccines are safe
They have been extensively studied, and no causal associations were found for multiple conditions, including Guillain-Barré syndrome (GBS), multiple sclerosis (MS) or other conditions that cause demyelination of nerves, chronic fatigue syndrome, arthritis, autoimmune disorders, asthma, sudden infant death syndrome (SIDS), alopecia or diabetes.

Hepatitis B vaccines work 
Between 1990 and 2015, use of hepatitis B vaccine in children around the globe was estimated to prevent 310 million new chronic hepatitis B infections. Between 2000 and 2019, hepatitis B vaccination of newborns and infants was estimated to prevent 22 million deaths globally from hepatitis B — about one death for every 13 vaccinations administered. Further, because deaths tend to occur decades after infection, it is estimated that between 2020 and 2030, another 16 million deaths will have been prevented by use of hepatitis B vaccine.

It did not find evidence that exposure to aluminum in vaccines had caused an increased risk for autoimmune, atopic or allergic, or neurodevelopmental disorders.
The work is by far the best available evidence on the question of the safety of aluminum in vaccines, said Adam Finn, a childhood vaccination expert in the UK and pediatrician at the University of Bristol, who was not involved in the study.

It's really hard as a pediatrician to deny giving a flu vaccine because we're out of stock of VFC flu knowing that it's a young infant's first flu season and we have plenty of flu vaccine for those with other insurance plans. Attached is an old pic from one of my fav ped FB pages. They did get some.

I fear that stopping COVID vaccination is the gateway to stopping all vaccinations. It won't end well, even if only this one type of vaccine is banned. This image from the article is fantastic! Just the type of stuff we need to promote to the general public to help them learn real risks.

As a doctor who has delivered over 5,000 babies, I can say with full confidence that boys are boys and girls are girls. This basic biological statement should not be radical, and it is sad that we are even having this debate.

80% of American agree with President Trump’s view that biological boys should not compete in girls’ sports. That’s why I was proud to cosponsor my good friend Coach Tommy Tuberville’s Protection of Women and Girls in Sports Act, legislation that would preserve Title IX protections for female athletes. Unfortunately, but not surprisingly every single Senate Democrat voted against it this week.

Thankfully, President Trump has taken up the mantle on this issue by signing an executive order last month protecting women’s and girls’ sports. I was honored to be a part of that historic moment and attend the ceremony as he signed the critical order. However, executive orders can easily be undone by future presidents. Congress must act to make this permanent and ensure women and girls are protected from the Left’s radical gender ideology. This week, I penned an op-ed in The Daily Caller highlighting the importance of protecting women’s and girls’ sports and calling out Senate Democrats for voting against the Protection of Women and Girls in Sports. Here are my 5 things I did and 5 worries.
Screenshot of an email to H R at OPM dot gov: I do not work for the government, but as a pediatrician I serve many families and am deeply affected by this current administration's sweeping changes. Five Things I Did Last Week: Counseled Kids and Teens with Mental Health Concerns: I saw many children and teens struggling with mental health who should be in therapy but can’t access it — either due to cost or because their families can’t take time off work or school. Over the years I have learned more about mental health therapy than I was taught in training due to need and the inability of patients to use mental health professionals due to costs and poor coverage with health insurance. In addition to counseling, I prescribed or renewed SSRIs and stimulant medications for many children and teens who rely on these medications to function each day. Discussed the importance of nutrition with many kids and teens, but some are such picky eaters that they would benefit from feeding therapy — an option their parents can’t afford or can’t manage due to work and school schedules. Talked to kids and teens about the importance of sleep. However, early school start times and their natural circadian rhythms make it nearly impossible for teens to get enough rest. Counseled many kids and teens about balancing screen time with physical activity and real-world social interactions. Unfortunately, extreme weather kept many of them home from school and activities, making screen time their default option. Vaccinated many children against vaccine preventable infections. I've counseled parents for years and have only recently had to handle questions about what they can do to protect their kids if vaccines become unavailable. Parents have enough anxiety to not have to worry if they will no longer be able to protect their children against deadly infections. Information is from the Institute of Medicine and Pless et al. Complication or Serious adverse event will be listed 1st, then risk after natural disease followed by risk after vaccination. Otitis media 7-9 per 100 infections, 0 vaccination. Diarrhea 8 per 100 infections, 0 vaccination, pneumonia 1-5 per 100, 0 vaccination, subacute sclerosing panencephalitis 4-11 per 100,000 infections, 0 vaccination, encephalitis 0.5 to 1 per 1000 infection, less than 1 per 1,000,000 vaccination, death approximately 1 per 1000 (1 to 15 per 100 in developing countries) infection vs 0 vaccine. Febrile seizures and thrombocytopenic purpura have been described but the risk is not well quantified after infections and 1 per 3000 after vaccination for febrile seizure and 1 per 30,000 after vaccination for thrombocytopenic purpura. Anaphylaxis has no risk after infection and 2-14 per 1,000,000 after vaccination.

Another TEACH Me ADHD training mission complete!

The detectives wrapped up their mission with a fun Family Mission: creating Glitter Jars to help settle their minds when emotions start swirling!

RFK Jr has called those who use antidepressants & ADHD medications addicts and is suggesting "wellness farms" to let people detox for a few years. Costs of these farms would apparently be covered by taxes on cannabis products. www.motherjones.com/politics/202...

In a podcast appearance last July, Kennedy said he planned to dedicate money generated from a sales tax on cannabis products to “creating wellness farms—drug rehabilitation farms, in rural areas all over this country.” He added, “I’m going to create these wellness farms where they can go to get off of illegal drugs, off of opiates, but also illegal drugs, other psychiatric drugs, if they want to, to get off of SSRIs, to get off of benzos, to get off of Adderall, and to spend time as much time as they need—three or four years if they need it—to learn to get reparented, to reconnect with communities.” The farm residents would grow their own organic food because, he suggested, many of their underlying problems could be “food-related.”

An ad to have blood testing was in my inbox today. It is wrong on so many levels! 1. You can learn your blood type for free by donating blood. This also helps others. 2. You can't change your blood type. 3. Healthy habits are good for everyone. They aren't recommended only with certain blood types.

Kennedy froze.

The color drained from his face.

“You’re asking me not to sue drug companies?” he said, voice rising.

“No, I’m not going to agree to that.”

Warren’s eyes gleamed like a hawk spotting a wounded rabbit.

“So you’ll be suing the same companies you’re supposed to regulate?”

Kennedy looked like he wanted to melt into his chair.

Then came Michael Bennet, a man who had been waiting patiently to drop a grenade into Kennedy’s lap.

"Did you say that Lyme disease was a militarily engineered bioweapon?” Bennet asked, deadpan.

Kennedy hesitated.

“I probably said that.”

“Did you say that pesticides turn children transgender?”

Kennedy turned bone white.

“I don’t recall saying that.”

Bennet’s lip twitched.

“But you do recall saying Lyme disease was a bioweapon?”

Kennedy looked like he had been hit by a tranquilizer dart.

Even the Republican senators were staring at their desks, avoiding eye contact.

The hearing finally adjourned, but Kennedy is not in the clear yet.

His next grilling is scheduled for tomorrow, and there’s no telling how much worse it can get.

His opponents smell blood. His supporters are already crafting conspiracy theories about the deep state.

And if the vote ends in a deadlock, Vice President JD Vance will cast the deciding vote.And finally, screen shot 7: Yes, JD Vance—the political equivalent of a wet cardboard box—will determine if a man once partially controlled by a brain parasite will run America’s health system.

Kennedy Jr. walked into his Senate confirmation hearing like a man stepping onto an ice rink wearing banana peels for shoes. He had one job: convince the world that he was not a bug-eyed conspiracy theorist who once hoarded a whale head and left a bear carcass in Central Park. Instead, he walked out as the leading cause of migraines among Democratic senators.

This was supposed to be his moment of redemption, his big I’m-not-actually-insane speech. Instead, it turned into a political demolition derby featuring protesters screaming that he was a liar and a killer, Bernie Sanders interrogating him about baby clothes, Elizabeth Warren asking if he planned to run HHS like a side hustle, and a surreal moment where Kennedy had to confirm that he probably said Lyme disease was a military bioweapon. By the end of the day, Capitol Police had forcibly removed more people from the chamber than a dive bar on St. Patrick’s Day.

Kennedy barely got through his opening statement before a woman exploded from the gallery like a jack-in-the-box filled with rage and science degrees.

“YOU LIE!” she screamed, holding up a sign that read VACCINES SAVE LIVES before being swiftly tackled and dragged out by Capitol Police.screen shot #2: Kennedy blinked rapidly, which is how you know he was hearing the voice of the worm that used to live in his brain whispering, Abort mission, Bobby. Abort mission.

A brief moment of peace settled over the room, and then it happened again.

“YOU'RE KILLING PEOPLE!” another protester howled, launching into a full-body rage spiral before security carried her out, legs kicking, like a screaming suitcase with opinions.

Kennedy took a deep breath and tried to regain his footing, but Senator Ron Wyden had been waiting for this moment like a prosecutor with a personal vendetta.

“Are you lying to us, Mr. Kennedy?” Wyden snapped, staring daggers at him.

Kennedy forced a nervous smile, but it came out looking like he’d just been told he had to fight a horse for a parking spot.

“That claim has been repeatedly debunked,” he said, attempting to sound reasonable despite an entire room full of people who were watching YouTube compilations of him saying the exact opposite.

Wyden wasn’t buying it.

“You signed a petition to restrict access to the COVID vaccine. Did you or did you not?”

Wyden was gearing up for a finishing blow when another protester detonated like a landmine.

“YOU’RE A FRAUD!” she shrieked as security dragged her away in a full-body lock.

Even the cops looked exhausted now.

Then came Bernie Sanders, a man who has not been in the mood for nonsense since 1972.

“Are you supportive of these baby onesies?” he demanded.

The room froze.

Kennedy’s brain crashed like a Windows 98 PC.

“Excuse me?”

Sanders lifted a printed-out photo of a baby bodysuit covered in anti-vaccine slogans.

“These are being sold by the Children’s Health Defense, the organization you founded.”

Kennedy looked like he had just accidentally eaten a ghost pepper and was trying to play it cool.screen shot #4: “I—I don’t have oversight over that organization anymore,” he mumbled.

Sanders cracked his knuckles like a man ready to fistfight a CEO and leaned in.

“Are you supportive of these onesies?”

Kennedy started sweating through his suit.

Laughter rippled through the room. A Republican senator actually covered his face.

Kennedy, now looking desperate for a fire alarm to pull, tried to pivot to his real passion: banning corn syrup.

Sanders wasn’t having it.

Then Elizabeth Warren took the mic, radiating pure prosecutorial energy.

“Will you commit to not taking money from pharmaceutical companies while serving as Secretary of Health?” she asked, in the tone of a woman who already knew the answer but was going to enjoy watching him squirm.

Kennedy grinned like a dog that just chewed up your furniture and is hoping you’ll laugh it off.

“I don’t think they’d want to give me money,” he chuckled.

Warren did not chuckle.

Our other presidential candidate apparently didn't dazzle. Why do people need a leader to dazzle? Didn't we learn in school that you can't know a book by its cover and it's the inside that matters? We're left with someone who apparently dazzled half of us, but the insides are rotten.
