DEPARTMENT OF HEALTH
protecting, maintaining and improving the health of il Minesotans
RABID BAT FOUND ON SIDEWALK
On Tuesday, September 15 at about 12pm, a bat was found on the sidewalk on Hennepin Ave between N. Washington Ave and N 3°St.
The bat died and was sent for rabies testing. This bat tested positive for
rabies.
A woman was seen crouching by the bat and putting it into a bag. She was described as a 25-to-30-year-old white woman, about 5'5", with curly brown hair and a curly-haired golden retriever-like dog. If this was you, please call 911 and ask to be connected to Animal Care & Control.
Bites from bats are tiny, may be painless, and may not leave a mark. An person who had physical contact or touched this bat should talk with health care provider this weekend for assessment. During the week, Minnesota Department of Health can also provide rabies assessments at 651-201-5414.
If vou did not touch or have any contact with the bat, you have no risk of rabies and do not need shots.

I find it increasingly annoying when I hear “we need pseudomonas coverage b/c patient is immunocompromised from diabetes” regardless of infection or DM severity!

My brother is Seattle said he fully funded the alternative public radio station during the pandemic, and they said “thanks! But you know we won’t ever play any of your music on the station, right?”

Unfortunately no. There were no linked diagnoses as these are not needed in hospice, so hard to track. Even harder to confirm, without UA, lab work, or imaging! My understanding is a lot will be for fever and/or acutely declining mental status. Truly a stewardship challenge

Patrick Crowley @pcrowleydo.bsky.social · 14/09/2026
Absolutely. I think there’s definitely a role in some cases (cellulitis in a patient expected to live another 3 months comes to mind) but the more I looked into practice, the more it looked like most were “just in case” with the assumption there could be few/no side effects!

What does this mean? Are antibiotics killing people? Probably not. But something changes that triggers the prescription, and I suspect it’s often advancement of the dying process

Patrick Crowley @pcrowleydo.bsky.social · 14/09/2026
Using a Landmark Analysis and time-dependent cox analysis to control for Immortality bias, we showed a new antibiotic prescription in hospice was associated with shorter time left alive

You can’t (or shouldn’t) enroll patients who are in hospice into a trial. Also not a great population for extensive surveys. But we can look at indirect effects - in this case time to death

Patrick Crowley @pcrowleydo.bsky.social · 14/09/2026
Super excited for our research to get published. 20-50% of patients enrolled in hospice will get an antibiotic, and we have little understanding of the effects of these #IDSky #MedSky

if we’re thinking of the same guy, I think I scrubbed in for a C-section with him. He spends half the year doing Doctors Without Borders so he told me about going through a Syrian city as soldiers cleared ISIS out. Like, 2 blocks behind the advancing line of attack giving med aid

The lab leak theory is not a strict scientific theory, but mostly a narrative of different allegations Rand Paul is not only falsely accusing Fauci of creating SARS-CoV-2, he is endorsing a very stupid version of the origin theory that has incredibly strong evidence against it. Let me illustrate:

when we were kids we never wore “safety gear” or worried about broken bones and we were fine. except for tim neely. tim jumped out of a treehouse and landed on his neck. i know he survived because there wasn’t a funeral but i don’t think his parents ever let him outside again. man i forgot about tim

I like that. One of our attendings at the VA would ask if we “have stock in these drugs?” when we suggested it multiple times on rounds

I used to tell them, unless you have stock in the prebiotic company, some yoghurt or kefyr will likely be as beneficial

Would love to hear others’ approach though. I’m sure I’m not the only one on #IDSky who has these conversations!

Ultimately, it’s their money. I respect that patients are just looking for something that will help with the very real side effects of the antibiotics. But I hope they don’t break the bank on ‘em
120
Patrick Crowley @pcrowleydo.bsky.social · 05/08/2026
So instead, I usually recommend food as medicine - fermented food, prebiotics - kombucha, Kimchi, sauerkraut, yogurt, kefir. These may help the good bacteria rebuild, but they are definitely way cheaper and people like eating them

If pharma could prove the benefits of probiotics, I guarantee these two would be sold for a lot more conditions!
130
Patrick Crowley @pcrowleydo.bsky.social · 05/08/2026
Second, if they did do what they say, they would probably be WAY more expensive Vowst and Rebyota are honest-to-god gut bacterial replacements used for recurrent C diff. They are between $5000-$10,000
120
Patrick Crowley @pcrowleydo.bsky.social · 05/08/2026
First, they often don’t have the bacteria they say, and if they do it is not in the numbers they say. And them being shelf stable is mostly a guess. As anyone who has worked in a micro lab can tell you, hard to propagate bacteria
120
Patrick Crowley @pcrowleydo.bsky.social · 05/08/2026
Usually I start by telling people I am skeptical of probiotics in general for 2 reasons
120
Patrick Crowley @pcrowleydo.bsky.social · 05/08/2026
One of the most common questions I get asked is whether to take a probiotic with an antibiotic. #IDSky #MedSky🧵
