I fully agree with you, and I genuinely think adopting a stronger attitude is the first step. In a majority of cases, I would say, it's the story of a game that players remember, and that is more easily IP ownable. Narrative should get sign-off, instead of having narrative glue pieces together

I was laid off shortly after The Final Shape, but this death march has been painful to witness even still. Folks don't understand how much exceptional talent combined into a great culture over there (at least, not on the executive level). I loved my time there. I feel for all those affected.

Ah no, the actual mechanic behind the loot cave was that low level mobs kept spawning in it when their predecessors were killed, quite quickly. So trash mobs quickly spawning in a chokepoint made for very easy killing - thus having a cave full of their loot.
