Highlighted passage: Included in this study were patients with biopsy-confirmed mNSCLC who had not yet progressed or experienced serious adverse events to single-agent chemotherapy, and thus had reasonable options for further palliative systemic therapy. Patients must have been discharged to a SNF with an intention for further therapy once functional status permitted. Fig 1. Of the total 427 patients, 173 (40.5%) had a hospital readmission within the subsequent 30 days after discharge to a SNF. Only 229 (53.6%) ever managed to return to an outpatient oncology visit, and only 131 (30.7%) received any subsequent systemic therapy. This proportion varied by the intended therapy (Fig 1; Table 2), with 40 of the 54 planned for a TKI (74.1%), 42 of the 130 planned for ICI (32.3%), and 49 of the 243 planned for cytotoxic chemotherapy (20.2%) receiving further therapy (P < .001).

excellent piece by @mirandayaver.bsky.social in @statnews.com on how health insurers use administrative burdens to "ration by inconvenience" require prior auth, deny claims -- "it's ok because they can appeal" but the appeal process is daunting, confusing, takes a ton of time and effort etc
I characterize this dynamic as “rationing by inconvenience.” The appeal process’s administrative burdens — to draw on the language of Pamela Herd and Donald Moynihan’s seminal work on this subject — lead far too many Americans to go without prescribed care not due to final denial, but rather because of accumulations of inconveniences and psychological toll leading people to abandon the appeal process. It may even make them more reluctant to seek future treatment.

not only are insurers effectively not liable for administrative burdens they place on patients an physicians as obstacles to care, but they are also incentivized by short-term savings even if it has long-term higher spending because they know people change insurance with jobs, age into medicare etc

In my research, I have characterized many of these insurance processes as "rationing care by inconvenience." That is, in addition to issuing frequent coverage denials (whether through prior authorization or other mechanisms), insurers impose administrative barriers to accessing care, whether in getting a prior authorization approved or appealing a denial.

Good news but need to see the HR and subgroups here, wish they hadn’t teased the “1-year” improvement in median OS. The interim OS HR was 0.8 and 5% diff at 2 years so curve probably very flat around the median meaning median diff can be huge with less impressive HR. #LCSM
