CNN ranks New Haven #2 among America's best towns to visit in 2026, citing its walkability, museums, music, theater, and, of course, a century of pizza. It's also a great place to train in nephrology! If you're considering training here, we encourage you to come see for yourself!

7/ Caveats matter here: the design is cross-sectional, and our ligand–receptor results are spatial co-expression, not proven signaling. What they give us is a set of testable targets — JAK-STAT inhibition, IFN-γ neutralization, complement, and the NAD+/NAMPT axis.
130
Megan Baker @meganbaker.bsky.social · 27/08/2026
6/ The regulatory brakes appear to be missing. Checkpoint-high CD8 tissue-resident memory cells were present in every reference kidney but absent from 7 of 8 AIN samples, and every patient above 50 predicted interactions/mm² lacked them entirely.
120
Megan Baker @meganbaker.bsky.social · 27/08/2026
5/ Those loops concentrate in lymphoid aggregates: 21 across 7 of 8 AIN patients, and none in reference tissue. CXCL9–CXCR3 signaling was 3.6-fold denser inside aggregates. Complement signaling was not — that stays at the tubular interface. Two distinct compartments.
120
Megan Baker @meganbaker.bsky.social · 27/08/2026
4/ The AIN-defining axis is interferon signaling. Predicted CXCL9–CXCR3 interactions were 44-fold higher in AIN than ATI, and 41% of them were CD8 T cell to CD8 T cell, meaning the chemokine network amplifies itself rather than only pulling cells toward tubules.
120
Megan Baker @meganbaker.bsky.social · 27/08/2026
3/ VCAM1+ injured proximal tubule cells are enriched 6.5-fold in AIN compared with acute tubular injury. They aren’t passive bystanders — they produce complement (C3, C4A, C4B, CFB), and C3AR1+ macrophages sit directly against them.
120
Megan Baker @meganbaker.bsky.social · 27/08/2026
2/ AIN causes 15–20% of biopsy-proven AKI, and management hasn’t moved in decades: stop the drug, try steroids, hope. Steroids fail in 20–40%. The infiltrate has been described for years, but the molecular circuits behind it haven’t.
