Dear Director Bhattacharya:

I saw your appearance on Fox News regarding the measles-associated deaths in my home state of Pennsylvania. When asked about these two deaths, you said 

"It's been striking to me to watch the Governor release some medical information, like the fact that the kids were not vaccinated. Of course, babies are not vaccinated for measles until they are at least 15 months old so, of course, they are not vaccinated." 

You then continued to accuse the Governor of playing this out over the airwaves and decreasing trust in public health, comparing it to (of course) the behavior of public health officials during the COVID pandemic.

Of course this RFI drops late on Friday. NIH proposes to stop providing specific scores and instead bin grants into 3 broad categories: most competitive, competitive, not discussed. A score would be calculated but not provided to the PI (or PO). grants.nih.gov/grants/guide...

Starting in Spring or Summer 2027 K99/R00 (under its new activity code KT1/R00) will only be available to US citizens, permanent residents, and non-citizen nationals. I am hearing this from many program officers, but NIH has yet to make an official statement to my knowledge.

The Director of the National Institutes of Health shall, as appropriate, encourage efforts to improve research related to the health of sexual and gender minority populations, including by—
(1) facilitating increased participation of sexual and gender minority populations in clinical research supported by the National Institutes of Health, and reporting on such participation, as applicable;
(2) facilitating the development of valid and reliable methods for research relevant to sexual and gender minority populations; and
(3) addressing methodological challenges.
(July 1, 1944, ch. 373, title IV, § 404N, as added Pub. L. 114–255, div. A, title II, § 2038(d), Dec. 13, 2016, 130 Stat. 1065.)(D) encourage the collaboration described in subparagraph (C) to—
(i) allow for an increase in the number of subjects studied; and
(ii) utilize diverse study populations, with special consideration to biological, social, and other determinants of health that contribute to health disparities;(A) identify strategic research priorities and objectives across biomedical research, including—
(i) an assessment of the state of biomedical and behavioral research, including areas of opportunity with respect to basic, clinical, and translational research;
(ii) priorities and objectives to advance the treatment, cure, and prevention of health conditions;
(iii) emerging scientific opportunities, rising public health challenges, and scientific knowledge gaps; and
(iv) the identification of near-, mid-, and long-term scientific needs;
(B) consider, in carrying out subparagraph (A)—
(i) disease burden in the United States and the potential for return on investment to the United States;
(ii) rare diseases and conditions;

Last year I looked at NIH grants for which the the titles had changed in the course of administrative renewal. These title changes were part of the negotiations required between program officers and applicants (a process referred to as "remediation" by Deputy Director Jon Lorsch). 1/5

Just today I talked to 3 scientists who are looking at closing their labs down. Terrible. I am an optimist - hope is an ethic and a choice! - but it’s important to be realistic that it’s going to get worse. And without a plan and organizing from scientists, things won’t fix themselves.

This line graph illustrates the percentage change in agency staff levels from the previous year for nine major U.S. federal scientific and health organizations between the fiscal years 2016 and 2025. The agencies tracked include the CDC, Department of Energy, EPA, FDA, NASA, NIH, NIST, NOAA, and NSF. For the majority of the timeline between 2016 and 2023, the agencies show relatively stable fluctuations, generally staying within a range of +5% to -5% change per year. However, there is a dramatic and uniform plummet starting in the 2024–25 period. Every agency depicted shows a sharp downward trajectory, with staffing losses ranging from approximately -15% to over -25%. The Environmental Protection Agency (EPA) shows the most significant decline, dropping to roughly -26%, while the National Institute of Standards and Technology (NIST) shows the least severe but still substantial drop at approximately -15%.

The Trump administration just announced the biggest restructuring of the US Forest Service in its 121 year history. Every regional office in the country is closing. 57 of 77 research stations are being shut down. Headquarters is moving from Washington DC to Salt Lake City, the capital of the state that has been actively suing the federal government to take control of your public lands. 16% of the workforce is already gone. This is not a reorganization. It is a dismantling.
