‘Official’ results from the Vertex VX-880 trial using stem cell-derived islets for people with type 1 diabetes #T1D 10 out of 12 people with reduced or eliminated need for insulin 🎉 But note the need for immunosuppression and limited eligibility… this is not (YET) the ‘cure for everyone’

Excellent to see this Breakthrough T1D and CIHR-funded research by Dr. Mahmud at SickKids (Toronto) that shows improved glycemic control and kidney function when using the drug dapagliflozin in youth. Exciting to see research progress toward adjunctive therapies for #T1D.

Such a ridiculous practice - same with NSERC. Always budget about 10% more than it will cost to hit closer to your target but not being ‘too high’. It’s a fine balance!

When beta cells make insulin, C-peptide is ‘broken off’ and this can be measured in blood draws. C-peptide has been the ‘primary outcome’ for many #T1D preventative therapy trials, but it’s not a very sensitive test and can be costly.

A biomarker is something measurable that indicates what’s happening in the body (an A1c is a biomarker of blood glucose). With an increase in screening tests and preventative therapies, biomarkers of beta cell function are becoming more important.

Great new research - one of the difficult aspects of #T1D is that there’s no way to “see” islet or beta cells to check if they are being attacked or functioning well so new biomarkers are important! (More info in comments…)

For those curious why the same cells are effective on one trial (VX-880) and not another (VX-264)… The 880 trial is putting the cells directly into the body and using immunosuppression drugs to protect them. The 264 trial was putting the same cells in a ‘teabag’ device that goes under the skin

Unfortunate to see today’s news from Vertex about their VX-264 therapy for #T1D but this is why we always have ‘multiple shots on goal’! Considering that the same cells are being used as in VX-880 (so far great data) it’s reasonable to assume that the device encapsulation was the problem here.

Always exciting to see new Clinical Practice Guidelines, but these are especially exciting: - T1D across the lifetime instead of broken up into stages 👏 - recommending AID for all ages and stages of T1D 👏👏 - first in the world to recommend considering the use of adjunctive therapies for T1D 👏👏👏

Take-home message: we need better trial design for preventative therapies... adaptive trial design is likely the best option, but #T1D is such a heterogenous disease and finding people in early-stage is difficult!

Really fascinating debate at #ATTD2025 about whether or not we should use placebos in trial design for preventative therapies of #T1D now that Tzield (teplizumab) has shown efficacy and is approved in a few countries. Heavy hitters Colin Dayan and Laura Jacobsen presented excellent points!
