Notably public disclosure in the Inquiry showed the Public Health England informed the UK IPC cell as early as December 2020 that the understanding of aerosol transmission had changed. This evidence supports airborne routes has always been a significant factor in the spread of Covid-19.

Scientific evidence: Urgent review of IPC guidance
12. We understand from your statement of 16 January 2025 that you would welcome an informed review of the current scientific evidence on routes of transmission.
13. We draw your attention to the enclosed Long Covid Groups joint letter endorsed by several other Core Participants seeking
urgent interim
recommendations that:
By 1 March 2025, UKHSA, NHSE, DHSC and other public health agencies (including those in the Devolved Nations), with the benefit of multi-disciplinary input from experts in physical sciences and the UK's Health and Safety regulator, the HSE, revise IPC guidance in the NIPCM and HTM guidelines to ensure:
(a) recognition of the role of airborne transmission of SARS-Cov-2; and
(b) there is appropriate guidance on measures to limit airborne transmission of respiratory viruses such as Covid-19 including the use of FFP3 masks, improved standards of ventilation and air filter devices in healthcare settings (both clinical and non-clinical) and ensure the distribution of those guidelines to all employers/providers of clinical and non-clinical healthcare
Any such review must include consultation with key stakeholders
14. We trust as Minister for health protection that you will want to take the opportunity support the call for interim recommendations to review IPO guidance in light of the evidence heard at the Covid-19 Public Inquiry
15. We would ask that you provide your response by 14 February 2025.

6. It is extremely troubling that the Government appears to be continuing to minimise the
role of airborne transmission rather than
focusing on
preventative measures such as investment in ventilation in public buildings including schools and hospitals.
Expert evidence: Airborne Transmission
7. The Covid-19 Inquiry experts to the Inquiry on IPC controls, Professor Clive Beggs, Dr Ben Warne and Dr Gee Yen Shin all agree that Covid-19 is transmitted through the airborne route. All agree that FFP3 respirators provide more protection for healthcare workers than surgical masks and there is an urgent need to update the IPC guidance to recommend the routine use of FFP3s and improve standards of ventilation and air filtration in the NHS estate to mitigate airborne transmission of Covid-19 and other respiratory viruses.
8. The evidence of Dr Ritchie was a significant outlier being inconsistent with all expert evidence adduced in the Inquiry.
9. Prof. Clive Beggs, explained to the Inquiry that Covid 19 is transmitted
"predominantly by an airborne route" and based on a range of evidence that he observed there was sufficient evidence that airborne transmission was a
"strong possibility'," even in the early stages of the pandemic and was unequivocal that the position on airborne transmission was clear by September 2020.
10. Dr Barry Jones remarked that SARS-CoV-2 "was defined as aerosol transmitted from the very moment it came into the country" and Covid 19 is said to mirror the airborne transmission patterns seen in related highly sequential infectious viruses like SARS-1 and MERS* (which you will of course be aware continue to be designated airborne viruses).
11. In addition to witnesses, the Scottish Chief Medical Office, Professor Sir Gregor Smith and Professor Dame J. Harries, the Chief Executive of the UK Health Security Agency (UKHSA) both point out that "aerosol transmission was very real" and accepts that its role was "significantly higher than (that) was accounted for.

1. We act for the Long Covid Groups in the Covid-19 Public Inquiry.
2. We write on behalf of the Long Covid Groups to express their concern regarding recent statements made by yourself asserting that Covid-19' is not predominantly transmitted via the airborne route.
3. This position appears to be at odds with the testimony of prominent officials and experts during the Module 3 Covid-19 Public Inquiry who have emphasised the significant role of airborne transmission.
4. By letter dated 16 January 2025 to Tim Farron MP, you referred to the October 2020 UK Government guidance on SARS-CoV2 routes
of
transmission and stated: "In her testimony to the Covid-19 Inquiry Dr Lisa Ritchie, NHS England's National Infection Prevention and Control Lead, reaffirmed that Covid-19 is not predominately transmitted through the airborne route. This position is supported by the Government and aligns with the scientific evidence provided by the World Health Organisation (WHO). Until scientific evidence suggests that the virus is predominately spread through the airborne route, this stance will remain unchanged."
