No use brow beating care staff to produce Hospital Passports if hospital staff do not read or act on them. No use repeatedly admitting a person for the same problem because community support is ineffwctive. 010

Health professionals need to listen better to people withlearning disabilities & the people who know them well - familes & long-term support/care staff. Continuity if care really matters. 110

Making sure there is equitable access to healthy lives, healthy food, enjoyable exercise, employment, relationships - bacause these things as much or more than health services are what keep us healthy. Expecting patient to remember date & name of operation + pathology findings is unrealistic. I'm sure you do read notes. Surprised how many don't. 021

My experience as a patient is that they don'tt know, & sometimes doubt, my past medical history. It seems it's my responsibility to remember . Also seems to be one of the worst for corridor, care which is not just Emergency Department waits but 'temporary' extra beds on almost every ward. 000
