@costplusdrugs.com - teduglutide (an actual FDA-approved GLP2 peptide) has gone off patent, but the current indication (short bowel syndrome) has a pretty small N. But at $400K per year, some competition on price would help a lot. And a lower price could inspire testing in new GI indications...

Look at this graph. The share of national income going to the richest 10 percent was lowest (and share to lower 90 percent highest) when union membership was at its highest. As unions have weakened, inequality has widened. This is why we must build back union power.

I refer a lot of patients to cpdrugs who have been cornered by their insurance and generic monopolies. It is too easy to manipulate a low margin generic drug to a high-margin monopoly for a small number of desperate patients and play the brutal "but what it is worth to YOU?" game. Market failure.

MC hastened to stick his name on it (after it became successful) so that he could get some reflected good PR. An investor, not his idea, not his execution. But it is filling a market hole and making reasonable money. Better margins than a grocery store.

The explosion of costplusdrugs shows that there was a market opportunity if you can scale - not just making one generic (entry costs, startup costs are big) but scaling to hundreds. People just did not recognize it. And doing some good in the market.

Certainly NOT efficient markets, and a need for government (CA) to step in. A strong case can be readily made for government (monopoly enforcement) to step in a lot more often - true in a lot of markets in the US. The example of Martin Shkreli suggests we need more regulation.

In theory, in he US system, a valuable med goes off patent and the invisible hand of capitalism leads generic cos to step in. Worked well for adalimumab (now 7 generics). Less well for uncommon meds like indomethacin suppositories. Low value, low margin. Only a single supplier at 50 cents each.
